What You’ll Do
As a Case Manager for the Residential Program, you will:
- To provide quality therapeutic case management services for adolescents and their families as part of a residential/day treatment team. Primary responsibilities are to act as a liaison and maintain communication between The Children's Home of Reading, the primary caregivers and referral/other agencies; represent the agency at all court hearings and interagency meetings; coordinate care services, discharge planning and transportation for residents and their families.
- Demonstrates strong communication, organization, time management, and clinical skills when working with adolescents experiencing mental health, emotional, or behavioral challenges.
- Provide psychoeducational groups, case management, supervision, and support to clients and families.
- Maintains accurate and timely documentation, including treatment plans, progress reviews, and discharge summaries.
- Coordinates services, appointments, transportation, and community resources for assigned clients.
- Maintains confidentiality and communicates effectively with families, referring agencies, treatment providers, and other community partners.
- Participates in treatment planning meetings, court hearings, and interagency meetings as required.
- Monitors client safety and supports treatment goals, behavioral plans, and individualized treatment objectives.
- Coordinates discharge planning and continuity of care for assigned clients.

Qualifications
- One to three years experience working in a human services field (CASSP system-related setting preferred).
- At least three (3) written references that demonstrate past educational and/or employment experience.
- Successful completion of core development orientation program.
- Must be at least 21 years of age.
- Valid PA driver’s license (or ability to obtain such license), and a good driving record, with proof of auto insurance coverage.
- PA Act 33/34 clearances.
- Pre-Employment physical from a licensed physician (after conditional job offer) verifying person has no communicable diseases or physical conditions that might endanger the health and safety of children or prevent the person from performing essential job responsibilities.
Schedule
