Position: Case Manager

Location: Torrington

Salary: $22.00/hour

Schedule: Full Time, 40 hours, 1st shift, Monday-Friday, with flexibility to meet the needs of the program.

 

Position Summary:

The Litchfield Next Steps Housing Case Manager provides comprehensive case management services to individuals with severe and persistent mental illness, co-occurring disorders, histories of homelessness and current housing instability. The Housing Case Manager provides initial and ongoing comprehensive assessments; regularly scheduled case management meetings and home inspections; coordination of and referral to community resources; and works collaboratively with clients on caseload to develop individual service plans in an effort to address housing stability and goals that reflect individual strengths, needs, abilities, and preferences.

 

Requirements:


Some post-secondary education is required with 5 years of related experience or a Bachelor’s Degree with 2 years of related experience
Knowledge of CAN System and local housing opportunities for low-income adults.
Experience working with the homeless and SPMI populations.
Valid Driver’s License


 

Orientation:

If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
