Job Summary Details

Provides clinical case management and nursing oversight for assigned Atlantic Private Care Services clients. Responsibilities include conducting prospective client evaluations, completing initial and ongoing assessments, developing and updating plans of care, coordinating and monitoring client services, and providing clinical supervision of HHA/CHHA, LPN & RN field staff assigned to client cases. Ensures care and clinical documentation are provided in accordance with the established plan of care, agency policies, and applicable regulatory requirements. Collaborates with clients, families, physicians, field staff, scheduling/service coordinators, and clinical leadership to support quality, continuity, and coordination of care.

Job Responsibilities / Essential Functions

  • Conduct prospective client evaluations and clinical screenings to determine service needs and appropriateness for Atlantic Private Care Services. 
  • Initiate services for assigned clients, including completing initial assessments, obtaining required consents and clinical information, and educating clients and families regarding available services and the plan of care. 
  • Secure physician orders and complete required clinical documentation including care plans, medication profiles, assessment, reassessment, admission & discharge paperwork.
  • Develop, implement, review, and revise individualized plans of care based on client assessments, identified needs, physician orders, and applicable standards of care. 
  • Coordinate and monitor services for assigned clients to promote continuity, quality, and appropriate delivery of care. 
  • Conduct required client monitoring visits, reassessments, and supervisory visits in accordance with agency policies and applicable regulatory requirements.  
  • Provide clinical supervision and evaluation of HHA/CHHA, LPN & RN field staff, including observation of care delivery and completion of required field staff evaluations to ensure services are provided in accordance with the client's plan of care, agency policies, and applicable regulatory requirements. 
  • Identify and document clinical or care-delivery concerns involving assigned field staff and communicate findings to the appropriate clinical leadership for follow-up as needed. 
  • Communicate the client's plan of care and changes in client needs or conditions to assigned field staff and other members of the care team as appropriate. 
  • For skilled nursing/private-duty nursing cases, establish and maintain required clinical records; obtain physician orders; complete medication profiles, assessments and reassessments; reconcile medications; and complete admission, ongoing, and discharge documentation as required. 
  • Provide clinical education and support to clients and caregivers regarding the plan of care, medications, health conditions, safety, self-care, and other identified needs. 
  • Provide clinical guidance and education to field staff related to client-specific care needs, clinical procedures, and identified learning needs. 
  • Collaborate with scheduling coordinators regarding client needs, field staff assignments, required supervisory visits, changes in client status, and continuity of services. 
  • Complete chart audits and other clinical documentation reviews as assigned to support quality, regulatory compliance, and performance-improvement activities. 
  • Complete required incident documentation and participate in follow-up related to client complaints, incidents, investigations, and service-recovery activities in collaboration with clinical leadership. 
  • Participate in case conferences, staff meetings, departmental meetings, performance-improvement activities, and other clinical initiatives as assigned. 
  • Comply with Atlantic Private Care Services policies and procedures and applicable federal, state, licensing, accreditation, and professional nursing standards. 
  • Provide weekend and evening RN on-call coverage as needed. 
  • Perform other clinical case-management duties as assigned within the scope of the position.

Qualifications

Required:

Education: Associate's degree in nursing. Bachelor's degree preferred
 

Experience:

At least 2 years’ experience in a home health care setting or revenant clinical setting
Experience working with home health care standards and quality assurance / risk management.
One year acute care nursing

Certificates: CPR
Licenses: Current license as a Registered Nurse the state of New Jersey or RN compact license
Meets the requirements of continuing education annually for license recertification
Meets or exceeds the requirements of the competency exams. Possesses certification and/or proof of a passing grade on a written exam.

Preferred:
2-3 years of previous Clinical Specialist experience preferred.
