FLSA DUTIES STATUS: Non-Exempt

REPORTS TO: Director of Key to Home

JOB OBJECTIVE: The Key to Home Early Intervention Case Manager will be a team member involved in a pilot program specializing in housing problem-solving, diversion, and rapid exit services. As a team member, the case manager will assist in successfully housing 30 households within 90 days. 


The essential job duties include but are not limited to:

  • Negotiating a return to previous housing.
  • Aiding and supporting with document collection, such as identification and other necessary documents.
  • Housing search assistance to relocate to a new housing unit.
  • Mediation with family, friends, or landlords to preserve existing housing.
  • Provide real-time assistance, including responding expediently, not exceeding one business day.
  • Providing short-term case management to resolve household housing crises.
  • Offer accessible services with flexible office hours.
  • Liaise with other service providers and community partners as a pathway to increase Diversion & Rapid Exit referrals and problem-solving conversations.
  • Demonstrate flexibility, consistency, and kindness when working with clients, CoC members, and partners
  • Completing flex-fund requests to facilitate financial assistance disbursement.
  • Completing accurate, timely, and complete HMIS data, including program enrollments, case notes, and services.
  • Adhere to the most up-to-date version of the Coordinated Entry Policies and CoC Standards of Care.
  • Participate in the CoC, regular ongoing engagement with the appropriate workgroups.

SUPERVISORY RESPONSIBILITIES:

There are no supervisory responsibilities for this position. 


Requirements

MINIMUM QUALIFICATIONS FOR CONSIDERATION:

  • Bachelor’s degree in social work, or human services related field required; master’s degree preferred and/or at least 12-month’s case management experience working with those experiencing homelessness 
  • Fluency in Spanish and English preferred

Knowledge, skills and abilities:

  • Strong verbal and written communication, listening, and presentation skills.
  • Experience with individuals experiencing mental health issues.
  • Strong, flexible interpersonal skills required to interact effectively with employees and managers at all levels in the organization and within the community partnership.
  • Ability to work effectively with staff, volunteers, parishes, and the community.
  • Ability to adjust communication style to handle diverse situations.
  • Excellent organizational and time management skills to ensure multiple tasks receive adequate and timely attention.
  • Demonstrated conflict resolution skills.
  • Able to function autonomously and be proactive.
  • PC literate and proficient in all Microsoft Office applications.
  • Ability to drive - must have a valid Oklahoma driver’s license

PHYSICAL REQUIREMENTS

  • High energy and stamina to actively engage with participants.
  • Lifting up to 40 pounds
  • Ability to respond quickly to crisis situations.
  • Ability to perform CPR and the Heimlich maneuver once trained.

WORKING CONDITIONS:

  • Must have a flexible schedule, evening hours required.
  • Over time when necessary to meet project deadlines.
  • Must be able to adjust to the environment of the target population 
  • May be subjected to fast-paced decision making, crisis situations.
