Under the supervision of the TCCOOMI Program Director, this position is responsible for the delivery of clinical services and community supports to persons with severe and persistent mental illness, as defined by the state authority, The Health and Human Services Commission (HHSC).  Services are to be provided according to the Texas Recovery and Resiliency (TRR) guidelines and must conform to the Performance Contract, Community Service Standards, TCOOMMI Program Guidelines, Helen Farabee Centers’ policy and procedures, and Medicaid standards.

This incumbent works closely with the Wise and Jack County Probation Offices in the provision of mental health services to consumers currently on Probation.  This position maintains an active, cooperative working relationship with Probation  Officers (PO) by assisting PO with necessary information exchange regarding clients, probation revocation, etc. Extensive fieldwork in the community is required while making contacts with consumers and maintaining caseloads and benchmarks set by HFC and TCOOMMI. Position is responsible for data input and maintenance of TCOOMMI web application. The incumbent formulates and updates treatment plans in accordance with the consumer’s needs, determines priority population, provides support, crisis intervention services, psychosocial rehab, and skills training.  The incumbent should have knowledge and skills in assessment, treatment planning, and community support services for adults with mental illness, as well as knowledge of psychotherapeutic medications and basic casework skills to assign services based upon the consumer’s diagnosis, level of need, individual treatment and support needs, focusing on specific individual preferences where reasonable and prudent.  The incumbent schedules uniform assessments and updates treatment plans every ninety (90) days and/or in accordance with consumer need.  The incumbent must provide face-to-face assessment and crisis intervention services to any person requesting help or presenting in crisis at any time. Teamwork, initiative and positive examples of work habits are always expected.  Must be able to work independently, while maintaining professionalism with consumers and community stakeholders.  This position requires an aptitude for working with severely mentally ill, often impoverished people; and for seeing this work as a clinical specialty in which we take great pride.  This position provides assistance keeping unassigned caseloads up to date and/or assistance to case workers with assigned caseloads (continuity of care (COC) services, transitional case management (TCM) services, and/or intensive case management (ICM) as needed).  The incumbent is accountable to the TCOOMMI Program Director.  HFC incorporates principles of trauma-informed care throughout the agency, emphasizing cultural sensitivity and a safe and respectful environment for all consumers and staff.  All job descriptions include the responsibility to learn about and implement trauma-informed practices within the scope of work for the position. Reassignment is a condition of employment that may occur at the employer’s discretion.  Your work location, hours, and duties could change based on program needs. 
QualificationsEducation, Training, and Experience

Minimum Requirements: Must have bachelor’s degree from accredited college or university with major in social, behavioral or human services field. Licensure is not required. Completion of training and work with special client populations, i.e. children, adults, or chemical dependency related problems. One-year full time paid mental health experience in a social service or treatment facility. Must have current driver’s license and be insurable under Center insurance. Must pass all Center-mandated trainings. Must also pass the TCOOMMI specific background check with TCIC/NCIC/FBI screening..

Knowledge, Skills, and Abilities


Ability to read and proof correspondence, reports and computer printouts manually, etc.
Good spelling and grammar skills.
Ability to write progress notes according to standard, assessments, brief mental status exams, memos, letters, various complex reports and miscellaneous other documents with acceptable spelling and grammar.
Ability to relate verbal instructions, communicate in a courteous and effective fashion with co-workers, consumers and families, agencies and the public.
Display a customer service orientation; at all times treating customers as equals and partners.
Skilled in the use of personal and network computers, word processing, database, spreadsheet and office management software (Microsoft Office). Experience with a web based Electronic Healthcare Record is a plus.
