What You’ll Do

As a Case Manager, you will provide intensive case management and supportive services to individuals in our residential treatment program who are experiencing homelessness and co-occurring disorders. You will help clients identify barriers, connect with resources, and work toward housing stability, wellness, and self-sufficiency.


Key Responsibilities

  • Conduct client assessments to identify needs, strengths, and barriers related to housing, income, wellness, and independent living.
  • Develop and update individualized service plans with clear and measurable goals.
  • Meet with clients regularly to review progress, address challenges, and provide ongoing support.
  • Connect clients with housing, benefits, healthcare, behavioral health, employment, and other community resources.
  • Coordinate services and advocate for clients with internal teams and outside providers.
  • Participate in case conferences, team meetings, case staffing, and community partner meetings.
  • Respond to urgent client needs and provide appropriate follow-up support.
  • Assist with housing placement, transition planning, discharge planning, and follow-up services.
  • Maintain accurate and timely case notes, client files, reports, and service documentation.
  • Enter client information into HMIS and other required databases.
  • Follow all agency policies, contractual requirements, and confidentiality standards.
  • Build positive working relationships with community partners and service providers.
  • Attend staff meetings, trainings, and supervision sessions.
  • Support special projects and other departmental needs as assigned.
  • Perform other related duties as assigned.

Requirements

 

What You’ll Bring 

  • Education: Bachelor’s degree in Social Work, Psychology, Human Services, or related field (equivalent experience may be considered). 
  • Experience: At least 2 years of case management, behavioral health, or social services experience, preferably with individuals experiencing homelessness and/or co-occurring disorders. 
  • Skills & Abilities: 
  • Strong organizational skills with the ability to manage multiple priorities. 
  • Excellent interpersonal and communication skills, both written and verbal. 
  • Cultural sensitivity and the ability to work effectively with diverse and vulnerable populations. 
  • Proficiency in Microsoft Office (Word, Excel) and data entry systems; HMIS experience preferred. 
  • Ability to work independently and collaboratively within a team. 
  • Demonstrated crisis management and problem-solving skills. 
  • Requirements: 
  • Valid Florida Driver’s License with clean driving record. 
  • Successful completion of toxicology screening, Level II background check, and OIG reference check. 
  • Ability to work a flexible schedule, including evenings, weekends, and holidays, as needed. 
  • Bilingual (English/Spanish or English/Creole) preferred. 

Physical & Work Environment Requirements 

  • Physical: Ability to stand, walk, bend, and lift up to 25 lbs.; may require occasional cleaning or physical activity to assist clients. 
  • Work Environment: Noise levels range from low to moderate; position is based on-site and not eligible for remote work.
