Salary Range: $24.00 to $26.00 hourly, DOE
What You'll Do: The Impact You'll Make
As an Interim Housing Case Manager, you’ll play a central role in our interim housing program, helping clients stabilize, connect with vital services, and prepare for long term housing. You’ll manage a caseload, conduct weekly check ins, support access to resources, and advocate for client needs using a trauma informed and harm reduction approach.
Client Engagement, Intake and Orientation
• Meet with new clients within 24 hours of program entry to complete intake, provide a tour, and explain program rules and services
• Build rapport using trauma informed and harm reduction approaches to create a safe, supportive environment
• Complete intake documentation within required timeframes and coordinate room assignments with site staff
• Confirm medication intake with medical team and support continuity of care
Ongoing Case Management and Housing Focused Support
• Conduct weekly check ins with assigned clients and document progress in CHAMP
• Support clients in obtaining critical documents such as IDs, benefits paperwork, and birth certificates
• Monitor client needs including mental health symptoms, substance use, and medication compliance
• Schedule and coordinate transportation for medical and housing related appointments
• Investigate concerns raised by staff and follow up with clients to address behavioral or engagement issues
• Collaborate with team members and attend multidisciplinary meetings to ensure coordinated care
Care Planning, Documentation and Compliance
• Develop and update individualized care plans aligned with client goals and needs
• Complete required assessments, maintain accurate records, and ensure compliance with agency and funder requirements
• Provide weekly progress updates to the Program Manager and contribute to service planning
• Perform additional duties as needed to support program success
What You'll Bring: Your Qualifications
Education and Experience
• At least one year of experience working with individuals experiencing homelessness and a bachelor’s degree in social work, psychology, or a related behavioral health field
• OR two years of relevant experience and a high school diploma or GED
• Experience supporting clients with co occurring mental health, substance use, or chronic health conditions is strongly preferred
• Familiarity with housing navigation, crisis response, and public benefits systems
Skills and Abilities
• Excellent communication, problem solving, and advocacy skills
• Highly organized with the ability to manage multiple priorities in a fast paced setting
• Proficient in using electronic health records, Microsoft Office, and case tracking systems
• Ability to work independently while collaborating within a multidisciplinary team
Licenses and Requirements
• Valid California Driver’s License, clean driving record, and insurability under agency policy if applicable
• Successful completion of Live Scan and background clearance
• Must be fully vaccinated for COVID 19 and have received at least one booster or provide approved exemption
