OVERVIEW

The Arizona Families in Recovery Succeeding Together (F.I.R.S.T.; AFF) program supports parents, caregivers, and families referred by the Department of Child Safety whose substance use has become a barrier to stability, reunification, and long-term success. Through a wide range of structured services, AFF helps individuals reduce or eliminate dependence on alcohol and drugs while also addressing challenges that impact family well-being and self-sufficiency. Services are available in outpatient, inpatient, and residential settings, including detoxification, methadone maintenance, and recovery support programs.

The Case Manager II Behavioral Health Tech (BHT) plays an important role by conducting initial assessments, coordinating care, and supporting clients in achieving their treatment and case plan goals. This position provides advocacy, follow-up assistance, referrals, and documentation to ensure quality services and contract compliance. With services delivered in homes, offices, and community settings, the Case Manager II BHT helps families access the resources they need to heal, reunify, and thrive.

ESSENTIAL FUNCTIONS

  • Receives referrals from the AFF coordinators and schedules and completes initial intakes and American Society of Addiction Medicine (ASAM) assessments within contractual timelines.
  • Assists with determining funding eligibility and enrolling clients with AHCCCS as needed.
  • Develops individual service plans in partnership with clients and the Adult Recovery Team (ART); monitors progress and implements follow-up activities.
  • Provides referrals, assists the client in arranging appointments with community resources or private group counseling sessions and monitors client progress.
  • Maintains communication with clients, AFF coordination, DCS and the ART team as contractually required.
  • Maintains individual service files for clients that include plans and strategies for achievement of goals and provides follow-up contacts to assess progress.
  • Provides recommended substance use disorder treatment in the form of individual or group substance use education, outpatient and intensive outpatient treatment.
  • May drive agency or personal vehicle on company business.

MINIMUM REQUIREMENTS

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Executive Director of Human Resources.

  • Associate Degree plus two (2) years of Behavioral Health work experience (AHCCCS requirement for BHT); or High School Diploma or GED plus four (4) years of Behavioral Health work experience (AHCCCS requirement for BHT)
  • Minimum of 2 years of experience working with adults and/or youth with substance use disorders providing peer support, case management, assessment, and/or substance use awareness education
  • Excellent data entry skills and experience utilizing Microsoft Office Suite
  • Excellent organization skills
  • Strong communication skills - oral and written communications are clear and concise using appropriate style, grammar, and tone

REGULATORY

  • Must be at least 18 years of age
  • Valid driver license, proof of insurance, and 39-month motor vehicle report
  • Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report (employer paid)
  • Pass pre-employment drug screen (incudes marijuana, regardless of recreational use laws) (employer paid)

DESIRED QUALIFICATIONS

  • Bachelor Degree in social service or related field
  • Minimum of one year of professional experience conducting comprehensive evaluations using the American Society of Addition Medicine (ASAM) criteria to determine the most appropriate level of care
  • Bilingual in English and Spanish, verbal and written
  • Additional years of directly-related experience
