Case Manager I SUD Focus

What we are looking for:

The Case Manager provides community-based case management services and support to Individuals with substance use disorders (SUD) and co-occurring conditions in the following areas: Self-Advocacy, Education, Community Awareness, Medication Support, and Management, using a recovery-oriented and harm-reduction-informed approach.

Compensation Range:

The starting pay for this position is $44,000 for unlicensed candidates, $50,830 - $70,542.50 for limited licensed candidates, and $52,593 - $75,315 annual salary for fully licensed candidates, and is based on non-discriminatory factors such as skills and experience.

• Provides community-based case management services with a primary focus on substance use disorder (SUD) treatment and recovery support, integrated with primary and behavioral health services
• Responds to the needs, desires, and interests of Individuals utilizing formal and informal assessment practices, including SUD-specific screening and assessment tools
• Provides weekly community-based interventions to Individuals
• Provides transportation assistance to Individuals served.
• Facilitates weekly recovery-focused groups for Individuals served
• Completes all required documentation completely, accurately, and in accordance with CNS policy
• Develops and implements Person-Centered Recovery/Treatment Plans for Individuals consistent with guidelines established by OCHN, DWIHN, MCBAP, and other funding and accrediting entities.
• Completes after-hours hospital pre-screening assessments
• Matches prevention, intervention, and resolution techniques to the particular circumstances of each Individual
• Identifies and uses effective advocacy strategies to address and overcome diverse challenges facing Individuals (e.g., human rights, legal, administrative, and financial), including barriers to substance use treatment access and recovery.
• Gathers and interprets information about substance use, dependence, or other addictive behaviors and develops treatment plans based on stage of change for identified problems and needs, implementing stage-wise interventions
• Uses motivational interviewing strategies to engage and support Individuals in the recovery process
• Utilizes a trauma-informed and harm-reduction-informed approach to deliver services
• Establishes collaborative relationship with Individuals

SELF-ADVOCACY/EDUCATION/COMMUNITY AWARENESS:

• Identifies and utilizes the resources and support necessary to assist Individuals in reaching goals relative to career, educational, and recovery pursuits.
• Matches specific supports and interventions to the unique needs of Individual Individuals and recognizes the importance of friends, family, recovery community, and community relationships. 
• Enhances the ability of the Individuals to lead a self-determined life in recovery by providing the support and information necessary to build self-esteem and assertiveness, to make decisions, and improve their quality of life
• Assists Individuals in identifying and gaining access to formal and informal recovery supports available in their community (e.g., mutual aid groups, recovery community organizations, sober living).

MEDICATION MANAGEMENT AND MONITORING:

• Assists in the coordination of medication for Individuals, including medications for opioid use disorder (MOUD) and other medication-assisted treatment (MAT), where applicable
• Coordinates and collaborates with primary and specialty care physicians, community health agencies, substance use treatment providers, and other health care providers
• Provides education about and monitoring for adverse reactions to medications
• Other duties as assigned

Qualifications:

 Required

• A Qualified Mental Health Professional (QMHP) or Qualified Substance Abuse/SUD Professional with one or more of the following degrees: Social Work, Psychology, Sociology, Criminal Justice, or a related field
• Working knowledge and understanding of substance use disorders, co-occurring mental illness, medications used in treatment, and experience with other interventions used to treat SUD and psychiatric disorders.
• Ability to share and teach on the continuum of drug use, such as initiation, intoxication, harmful use, abuse, dependence, withdrawal, craving, relapse, and recovery
• Ability to share and teach about the symptoms of substance use disorders that are similar to those of other medical and/or psychological disorders and how these disorders interact
• Ability to recognize the importance of collecting and reporting on outcome data
• Ability to apply methods for measuring the multiple variables of treatment outcome
• Working knowledge of applicable Federal, State, and Local laws and regulations, the CNS Corporate Compliance Program, CNS Code of Ethics, as well as other policies and procedures.
• Ability to communicate effectively, professionally, and courteously
• Ability to use discretion and judgment when handling matters of a sensitive or confidential nature
• Advanced time management, problem-solving, customer service, interpersonal, and conflict resolution skills
• Advanced collaboration and team working skills.
• Ability to be forward-thinking and take initiative to accomplish goals and objectives of the department
• Intermediate-level experience with computers and Microsoft Office products

Preferred 

• One year paid or supervised volunteer experience in Health or Human Services, preferably in substance use disorder or recovery-oriented services.
• State licensure preferred; LLBSW, LBSW
