The purpose of this position is to provide comprehensive case management services to clients ensuring the delivery of high-quality, person-centered services consistent with the Agency's mission, policies, and programs.

Knowledge, Skills and Abilities:

  • Performs a range of social work duties, including gathering and analyzing data and providing services that match client needs, assisting with applications for public benefits and formulating service plans as appropriate.
  • Accompanies clients to medical, public benefits, and other appointments.
  • Assesses client needs and coordinates referrals for employment, English language, legal, and other services.
  • Develops and implements skills in providing appropriate services with increasing responsibility for the appraisal of the effectiveness of own work.
  • Performs record-keeping functions and other administrative duties as required by the Agency.
  • Participates in agency staff development programs and professional group seminars or conferences to increase professional knowledge and to report on own work.
  • Manages emergency situations.
  • Carries out an increasing role in interpreting the Agency to the community.
  • Collaborates with other agency personnel, supervisor, appropriate specialists or consultants on specific case-related problems.
  • Represents JCFS and its programs in a positive manner to both internal and external contacts.
  • Performs other duties as assigned.

Requirements:

  • Bachelor's degree in social work or a related human services field preferred; or equivalent combination of education and relevant work experience will be considered.
  • Valid Illinois driver’s license, a safe driving record, and regular access to a reliable, insured vehicle.
  • Ability to work a flexible schedule including weekend and evening hours when necessary.
  • Proficiency with Outlook, Word, Excel, Teams and other basic computer programs.

Preferred:

  • Fluent in English and at least one additional language representative of the clients served.
