Guthrie Home Health New York is hiring a Full-Time RN Case Manager to join our Cortland/Waverly based team. You’ll provide skilled nursing care to patients in their homes, helping them recover safely following hospitalization, illness, or injury while maintaining their independence.
This role is a great fit for an experienced RN who is confident working independently, values continuity of care, and enjoys building relationships with patients and families. You’ll manage your daily schedule with flexibility and accountability while receiving support from a collaborative Home Health team.
About the Role
- Schedule: Monday–Friday, primarily 8:00 AM – 4:30 PM
- Flexibility: Daily scheduling flexibility based on patient load
- Work Environment: Independent fieldwork with strong team collaboration and support
- On-call and weekend rotation with our other Case Managers

Position Summary:
Responsible for the case management and provision of quality home nursing services to patients within the agency’s philosophy, policy, and Standards of Community Health Nursing Practice.
Education, License & Cert:
Graduation from an approved School of Nursing, Bachelor’s Degree in Nursing preferred.
Experience:
One‐year previous nursing experience with Medical/Surgical Nursing required. Prior Public Health/Community Health experience preferred.
Essential Functions:
- Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient’s needs are met and quality care provided in accordance with Federal, State, and program guidelines.
- Is responsible for ensuring that he or she can meet the yearly competency requirements. Attends staff meetings and educational opportunities.
- Must provide nursing care in the patient’s environment.
- Manages the activities of the multi‐disciplinary team providing care to patient.
- Demonstrates effective time management, flexibility and priority setting in patient care.
- Must have excellent verbal and nonverbal communication skills and facilitate communication between team members and patients.
- Independent judgement and excellent planning and problem‐solving skills are required.
- Is responsible for LPN supervision.
- Must have reliable transportation.
- Must be able to accept on call assignments.

- Travel for this position is always required.

The pay range for this position is $83,200 to $112,569.60 annually.
