Position Summary
Interdynamics, Inc. is seeking a dedicated and compassionate Case Manager to support the delivery of Outpatient Treatment Services (OTS) under its contract with the Court Services and Offender Supervision Agency (CSOSA). The Case Manager serves as the primary coordinator of care for participants enrolled in the program and is responsible for ensuring that individuals receive comprehensive, person-centered services that promote recovery, stability, and successful community reintegration.
The Case Manager plays a vital role in the participant's treatment journey by conducting intake activities, coordinating services, developing Individualized Care Plans (ICPs), facilitating discharge planning, and maintaining communication with Community Supervision Officers (CSOs) and other stakeholders. This position requires strong organizational skills, a collaborative approach, and a commitment to serving justice-involved populations with professionalism and empathy.
The ideal candidate possesses experience in behavioral health, case management, and care coordination and demonstrates a strong understanding of substance use disorders, recovery principles, and evidence-based practices.
Essential Duties and Responsibilities
Participant Intake and Engagement
● Conduct intake interviews with participants within seven (7) days of receiving a CSOSA Billing Authorization.
● Coordinate participant admissions and ensure timely enrollment into treatment services. ● Conduct program orientations and explain participant rights, responsibilities, and program expectations.
● Distribute program handbooks, schedules, and relevant program materials. ● Obtain and maintain required releases of information and confidentiality documents.
● Notify Community Supervision Officers (CSOs) regarding participant admissions and attendance status.
● Assist participants in understanding treatment requirements and available support services.
Care Coordination and Case Management
● Coordinate behavioral health, medical, housing, employment, educational, and vocational services for participants.
● Facilitate referrals to internal and external service providers.
● Monitor participant engagement and address barriers to treatment participation. ● Assist participants with completing applications and accessing community resources. ● Coordinate appointments and follow-up services to ensure continuity of care. ● Participate in interdisciplinary team meetings and case staffing sessions. ● Collaborate with counselors, Clinical Supervisors, and CSOSA personnel to ensure comprehensive care coordination.
Treatment Planning
● Develop Individualized Care Plans (ICPs) based on assessment findings and participant needs.
● Identify participant strengths, challenges, and recovery goals.
● Document treatment objectives, interventions, and anticipated service schedules. ● Ensure treatment plans are person-centered, medically necessary, and aligned with ASAM Criteria.
● Update treatment plans in response to changes in participant needs or significant life events.
● Coordinate with the Clinical Supervisor to obtain required approvals and signatures for all treatment plans.
Assessment and Documentation
● Assist in coordinating behavioral health assessments and ongoing evaluations. ● Ensure all required clinical documentation is completed accurately and within established timelines.
● Maintain complete and organized electronic treatment files.
● Document participant encounters, interventions, and service coordination activities. ● Ensure all records comply with HIPAA, 42 CFR Part 2, and CSOSA requirements. ● Maintain participant confidentiality and safeguard protected health information.
Participant Monitoring and Support
● Monitor participant attendance and treatment compliance.
● Contact participants following unexcused absences to promote re-engagement in services.
● Identify participant needs and coordinate interventions to address emerging concerns.
● Assist participants in developing life skills and recovery support strategies. ● Support participants through transitions in care and discharge planning. ● Provide encouragement and advocacy to support participant success throughout treatment.
Discharge Planning and Aftercare
● Coordinate discharge planning activities in collaboration with treatment staff and CSOSA personnel.
● Prepare discharge reports and discharge summaries within established timelines. ● Develop individualized aftercare plans and recommendations for ongoing support services.
● Ensure participants are connected to community resources and follow-up services upon program completion.
● Participate in discharge staffing meetings and document participant progress and outcomes.
● Maintain records related to participant discharges and aftercare planning. Stakeholder Collaboration
● Maintain effective communication with:
○ Community Supervision Officers (CSOs)
○ Clinical Supervisors
○ Substance Abuse Counselors
○ External service providers
○ Community partners
● Participate in interdisciplinary team meetings and contribute to treatment planning discussions.
● Serve as an advocate for participant needs while supporting program and contractual requirements.
● Ensure timely communication regarding participant status, attendance, and treatment progress.
Minimum Qualifications
● Master's degree in Social Work, Psychology, Counseling, Rehabilitation Counseling, Family Counseling, or a related health and human services field from an accredited institution.
● Current licensure or certification as one of the following:
○ Licensed Independent Clinical Social Worker (LICSW)
○ Licensed Clinical Professional Counselor (LCPC)
○ Licensed Marriage and Family Therapist (LMFT)
○ Certified Addictions Counselor (CAC Level II)
○ Equivalent credential recognized by the District of Columbia or another U.S. jurisdiction
● Experience in behavioral health case management, care coordination, or substance use treatment preferred.
● Experience working with justice-involved populations is highly desirable.

Work Environment
This position operates in a hybrid environment and requires regular interaction with participants, program staff, and CSOSA personnel. The Case Manager may be required to attend meetings at CSOSA locations and community partner sites throughout the Washington metropolitan area.
