Hours: 30-40 hours per week, Monday - Friday 8:00 - 5:00
CHA is hiring a highly motivated, creative, and flexible individual to provide client support and case management across various locations including clinical, community, detention and harm reduction settings.
Primary Responsibilities:
- Provide case management
- Conduct intakes and enrollment activities
- Follow care plans for clients
- Complete data tracking and monthly reports
- Document appropriately in electronic health record
- Accompany clients to meetings and participate in meetings of community groups to advocate client needs
Education/Experience:
- Bachelor's degree in a human service field or similar degree with two years of experience with incarceration, probation, substance use recovery, mental health treatment or homelessness OR
- High school diploma or equivalent with 4 years of experience with incarceration, probation, substance use recovery, mental health treatment or homelessness and a peer support specialist certification OR equivalent education or experience.
- Lived experience preferred.
Knowledge, Skills & Abilities:
- Knowledge of harm reduction principles, recovery and person first language
- Ability to plan and work collaboratively with community partners and CHA staff in person and virtually
- Ability to work with diverse groups of people and to deal tactfully with others
- Ability to exercise good judgement in appraising situations and making decisions
- Accurately record services rendered and to interpret and explain records
