Overview


HOPE Community Services, Inc. is a leading outpatient treatment provider in south Oklahoma City.
We provide compassionate trauma informed care to individuals whose lives have been impacted by mental health and substance use disorders. We do this by providing compassionate and evidence-based treatment, aimed at improving the quality of life for those we serve.
 

We are seeking a dedicated Case Manager to join our Engagement team. In this role, you will support individuals in accessing services, gaining independence, and remaining stable in their community. About the Position 

As a Case Manager, you’ll provide direct services to individuals needing regular support to maintain community living. You will coordinate care, assist with independent living skills, and connect clients with vital resources like housing, benefits, and healthcare. Your work will include in-office and community-based engagement, building trust with clients and helping them move toward long-term recovery and self-sufficiency. 

Case Managers play a key role in the early engagement process, ensuring new clients receive at least four services within their first 45 daysYou’ll support clients in setting personal goals, securing basic needs, and navigating the behavioral health system—all while documenting services thoroughly and on time. 


About the Program 

The Engagement team within our Access and Intake Program helps individuals experiencing behavioral health or co-occurring needs begin their recovery journey. This team builds relationships with new or re-engaging clients and supports them in stabilizing their lives. The Access and Intake Program creates a welcoming and recovery-focused environment that emphasizes rapid connection to care, advocacy, and coordination of services. Staff provide skill-building, resource linkage, and trauma-informed support to promote independence and reduce barriers to treatment. 


Requirements 

  • Bachelor’s degree in psychology, social work, or related behavioral health field

     
  • Must obtain and maintain DMHSAS Case Management certification

     
  • Reliable Transportation
