As a Case Manager, you'll play an important role in helping clients navigate healthcare, social services, legal systems, employment, education, and other community resources. You'll work alongside treatment teams, families, and community partners to ensure clients receive coordinated support that addresses their individual needs and goals.

What You'll Do
The Case Manager provides comprehensive case management and support services to clients participating in substance use disorder treatment programs. The position works collaboratively with treatment teams, community partners, families, and other support systems to promote recovery, independence, and long-term stability.
Client Advocacy & Support
Your responsibilities may include:
Advocate for clients navigating healthcare, legal, social service, and other systems.
Help clients access needed medical, psychological, social, educational, and vocational services.
Assist clients with obtaining public benefits and other community resources.
Identify unmet needs and help develop appropriate solutions.
Support clients in achieving goals related to independent living, employment, education, community involvement, and meaningful leisure activities.
Help clients develop and strengthen natural support networks.
Maintain regular contact with clients through in-person meetings, telephone calls, or telehealth sessions.
Provide support and encouragement while maintaining appropriate professional boundaries.
Youth, Family & Community Engagement
Support youth-focused services and school partnerships as assigned.
Assist with family engagement and client-identified support networks.
Maintain strong community connections within Region 7.
Communicate with family members and support networks when appropriate and with client consent.
Serve as a liaison with external agencies, community organizations, and service providers.
Recovery Support & Groups
Facilitate or co-facilitate recovery-oriented groups and psychoeducation sessions as needed.
Support individuals in developing skills and connections that promote recovery.
Encourage participation in meaningful community activities.
Provide recovery-oriented support consistent with the client's treatment plan and identified goals.
Treatment Team Collaboration
Participate in treatment team meetings for assigned clients and individuals requesting support services.
Collaborate with counselors, medical staff, Team Leaders, probation and parole officers, and external providers.
Communicate client needs and concerns to the appropriate professionals.
Appropriately route client concerns to the appropriate resource or professional.
Complete assignments and tasks delegated by the Team Leader.
Participate in program development and quality improvement activities.
Crisis Support
Provide on-call crisis intervention services as scheduled.
Respond appropriately to client concerns and changing circumstances.
Connect clients with appropriate emergency, clinical, or community resources when necessary.
Maintain professional boundaries and follow agency crisis-response procedures.
Documentation & Compliance
Maintain accurate, timely, and confidential client records.
Complete progress notes, reports, and client file updates.
Document services in accordance with agency standards and applicable requirements.
Track and report client outcomes, program metrics, and assigned performance measures.
Maintain compliance with HIPAA, 42 CFR Part 2, agency policies, funding requirements, and ethical standards.
Participate in data collection, quality improvement activities, and program evaluation.
Maintain confidentiality and professional boundaries at all times.

Qualifications
Education & Experience
Preferred:
Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field.
One to two years of experience in case management, counseling, or human services.
Experience working with individuals with substance use disorders, co-occurring mental health conditions, or in recovery-oriented settings.

The position may involve travel throughout the community, including driving to client appointments, partner agencies, and community resources as required.
