Case Manager – Neurorehabilitation

Job Type: Full-Time
Schedule: Monday–Friday, 40 hours per week

Special Tree Rehabilitation System is seeking a compassionate and experienced Case Manager to join our NeuroCare Campus in Romulus, Michigan.

In this role, you’ll work with individuals recovering from brain injuries, spinal cord injuries, strokes, and other neurological conditions. You’ll help coordinate care, support discharge planning, connect individuals and families with community resources, and advocate for successful transitions to home and community living.

What We’re Looking For

Required/Preferred Qualifications:


Bachelor’s degree or higher in Social Work, Nursing, Psychology, Human Services, or a related field
Current Michigan RN license or LBSW/LMSW preferred
Certified Case Manager (CCM) certification preferred
Previous experience in case management, care coordination, discharge planning, long-term care, Adult Foster Care (AFC), skilled nursing, neurorehabilitation, or other post-acute healthcare settings
Knowledge of community resources and healthcare systems
Strong communication, organization, advocacy, and problem-solving skills
Ability to work effectively as part of an interdisciplinary team
Compassion and a genuine commitment to helping individuals and families


What You’ll Do


Coordinate individualized services and care
Develop and facilitate discharge plans and transitions of care
Advocate for client needs, preferences, and goals
Participate in interdisciplinary care planning
Coordinate with physicians, nurses, therapists, families, guardians, payers, and community providers
Connect clients and families with appropriate community resources
Assist clients and families in navigating the transition back to home and community settings
Communicate with external agencies and service providers
Support safe and successful discharge outcomes


Compensation & Benefits

Annual Compensation: $60,000–$75,000+, depending on credentials and experience.
