Enloe Health Rehabilitation Center is a 40-bed acute rehabilitation unit dedicated to helping patients who are recovering from various injuries or illnesses. In partnership with Lifepoint Rehabilitation, this facility provides 24 hours of nursing care and intensive inpatient therapy to provide an outstanding experience and quality outcomes for all of the patients we serve. 

How you'll contribute 

A Case Manager who excels in this role:

• Plans, coordinates, and evaluates patient care and movement throughout the continuum of care.
• Conducts patient assessments and identifies appropriate care options and community resources.
• Develops and supports individualized care plans in collaboration with the interdisciplinary treatment team.
• Establishes and maintains therapeutic relationships with patients and families throughout the rehabilitation process.
• Provides patient and family education regarding resources and services that support continued functional independence.
• Coordinates treatment activities and communicates with physicians, clinical staff, and other members of the interdisciplinary team.
• Obtains pre-certification and re-certification of insurance coverage in accordance with established policies and procedures.
• Collects and utilizes patient and quality data to evaluate outcomes, improve care coordination, and support quality improvement initiatives.
• Advocates for appropriate utilization of resources and assists with transitions to alternate levels of care when appropriate.
• Participates in quality management, safety, professional development, and program improvement activities.

What we're looking for 

• Education: Master’s degree in Social Work as required by the state.

• Licensure: Current applicable state licensure as a Master’s Social Worker. Registered Nurse or other licensed professional/certified case manager may be acceptable based on position requirements.

• Experience: Minimum of 3–5 years of clinical experience, preferably in medical-surgical and post-acute care settings.

Pay wage: $32.64-$59.16
