What You'll Do

As a Case Manager, you will:

Coordinate medical care and treatment services for youth in care
Maintain medical records and ensure compliance with regulatory requirements
Schedule and coordinate routine, specialty, psychiatric, and follow-up appointments
Assist with transportation to medical appointments as needed
Monitor medication administration and complete weekly medication counts
Train staff in medication administration, CPR, and First Aid procedures
Communicate medical updates and treatment information with parents, guardians, providers, and treatment team members
Collaborate closely with clinicians, direct care staff, families, and community partners to support positive outcomes
Maintain accurate documentation and electronic records
Assist with program operations and provide support coverage when necessary
Support quality care initiatives and contribute to overall program success
Perform additional duties as assigned

Qualifications

Required

Bachelor’s degree and one year of experience preferred OR equivalent combination of education and

experience.

• Intermediate computer skills

• Have the ability to establish a HIPAA compliant home office

• Valid driver’s license.

· Must pass drug screening and state and federal background checks

Preferred

Previous experience in behavioral health, human services, case management, or residential care settings
Experience working with adolescents and families

Schedule

Full-Time
Monday-Friday

Physical Requirements (If Applicable)

Ability to lift up to 25 pounds
Ability to stand, walk, bend, and assist youth as needed
Ability to perform essential job functions with or without reasonable accommodation
