Join Wasatch Behavioral Health as a Full-Time Case Manager with our WATCH Program (Wasatch Assistance Team Counseling the Homeless) in Provo, Utah! This rewarding opportunity offers a competitive wage of $21.36–$29.88/hour and the chance to make a meaningful difference in the lives of individuals experiencing homelessness and serious mental illness. Based at the Food & Care Coalition, the WATCH Program provides outreach, case management, therapy, medication management, and connections to essential community resources. As a Case Manager, you'll help clients navigate barriers, access critical services, and build the skills and supports needed to achieve greater stability, independence, and recovery.

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Compensation: $21.36–$29.88 DOE and Licensure

What You'll Do as a Case Manager

  • Conduct outreach and build supportive relationships with individuals experiencing homelessness.
  • Complete assessments and help develop individualized service and recovery plans.
  • Connect clients with community resources, including housing assistance, Medicaid, Social Security benefits, food assistance, and other essential services.
  • Coordinate care with therapists, prescribers, nurses, and community partners to support client success.
  • Monitor client progress and provide ongoing case management, advocacy, and problem-solving support.
  • Assist clients in working toward treatment goals, increased independence, and long-term stability.
  • Maintain accurate documentation and ensure compliance with agency and Medicaid requirements.
  • Promote recovery-oriented, trauma-informed, and person-centered services in every client interaction.

Qualifications

Applicants must meet ONE of the following:

Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.

  • Bachelor's degree with Social Service Worker (SSW) license
  • Bachelor's degree in any field with willingness to pursue SSW licensure

Additional Requirements:

  • Valid driver's license
  • Basic understanding of Social Work principles
  • Familiarity with social welfare policy and human development
  • Ability to problem-solve and maintain a client-centered approach
  • Must pass the Utah County Jail's criminal background check.
  • Spanish speakers may qualify for increased wages after testing

Employment at Wasatch Behavioral Health is contingent upon successful completion of a drug screen, motor vehicle record review and criminal background check. After hire you must complete and receive a Criminal Background Clearance (BCI) from Utah State Office of Licensing. You will additionally be required to pass the Utah County Jail's criminal background check. Upon receipt of a conditional offer of employment in the position, you must provide an official copy of licensure or certification.
