Position Overview & Organizational Relationships
Under the direct supervision of the Director of Marillac, the Marillac Case Manager is responsible for assisting families with all facets of case management and housing stabilization. This includes conducting intakes and assessments, developing individualized service plans, coordinating community support services, and facilitating housing retention activities. The Case Manager works cooperatively with Collaborative agency staff and maintains a collaborative, team-oriented approach across all levels of Marillac Shelter personnel.
Essential Duties & Responsibilities

Intake, Assessment & Service Planning:
- Conduct comprehensive assessments to independently identify family strengths and needs.
- Develop, monitor, and update individualized service plans in accordance with NYS OTDA requirements to ensure forward progression toward family goals.

Direct Case Management & Life Skills Support:
- Provide intensive case management to stabilize conditions contributing to homelessness, offering guidance in self-sufficiency, home maintenance, budgeting, meal preparation, time management, parenting skills, and tenant rights.
- Assist families in identifying and overcoming barriers to housing retention by connecting them with essential rehabilitative, mental health (including trauma-informed care), educational, employment, and substance abuse services.
- Coordinate transportation for families to medical, social service, and apartment-search appointments as appropriate.

Advocacy & System Navigation:
- Serve as the primary advocate and liaison for families when navigating public assistance, housing resources, and community service networks.
- Empower clients to make informed daily choices, assisting them in building skills and confidence to achieve long-term housing independence.

Crisis Intervention & Mediation:
- Intervene in crisis situations and facilitate conflict resolution, providing mediation between landlords and tenants or within facility settings to maintain housing stability.

Facility Safety & Facility Coordination:
- Assist facility staff in service delivery to ensure a safe, secure, clean, and healthy living environment; report operational concerns through appropriate channels.

Documentation, Compliance & Quality Improvement:
- Maintain timely, complete documentation including progress notes, incident reports, assessment forms, and statistics in case files.
- Complete required outcome measures for Quality Improvement (QI) and submit all administrative reports on time.

Professional Standards & Collaboration:
- Establish and maintain respectful, professional working relationships with clients and colleagues while adhering strictly to personal boundaries, agency policies, and client confidentiality.
- Participate in regular professional development, attend shelter staff meetings, and clearly communicate program updates or concerns to the Program Director.
- Perform additional position-related duties and responsibilities as assigned by the Director.
Requirements & Qualifications
- Schedule Flexibility: Monday – Friday, 9:00am – 5:00pm, with the flexibility to work outside regularly scheduled hours as needed (including evenings, weekends, or rotated holidays).

Education & Experience:
- Associate degree in a related field with two years of relevant experience; BSW or Bachelor’s Degree in a Human Services field preferred.
- Will consider previous professional and/or life experience in lieu of a college degree (High School Diploma or GED equivalent required).
- Previous experience in case planning or case management preferred.
- Previous experience working in a human services agency is a plus!
- Advocacy & Knowledge: A professional record of integrity and strong advocacy for homeless families, with familiarity with public assistance and other local resources.
- Driver & Vehicle Requirements: Must possess a current, clean, and valid New York State Driver’s License (actively licensed for at least one year), a reliable vehicle for transporting clients to community resources and meetings, and proof of 100/300 liability insurance coverage. Must be at least 21 years of age and meet agency insurance criteria for authorized drivers.
- Computer Skills: Proficiency in computer applications, such as Microsoft Office & Google Workspace Suites, and secure internet practices.
- Work Environment & Proximity: Must be willing and able to work in close proximity with the clients served.
