The Eagle Mountain Family Clinic provides a variety of therapeutic services for clients affected by trauma and/or mental illness. As a Case Manager, you'll help clients access community resources, coordinate services, and connect with the support they need while working alongside a collaborative multidisciplinary team to help clients build stability, confidence, and progress toward their personal goals.
Schedule: Monday-Friday, Hours are Varied
Compensation: $21.36β$29.88 DOE and Licensure

What You'll Do as a Case Manager
- Conduct client assessments and help create individualized care plans.
- Coordinate services and collaborate with other providers to support client needs.
- Offer ongoing support, guidance, and follow-through with clients.
- Connect clients and families with community resources and supportive services.
- Work closely with a supportive, multidisciplinary team.
- Help clients navigate appointments, services, and other resources.
- Build positive relationships with clients and families through compassionate, client-centered support.
- Help clients build stability, confidence, and progress toward personal goals.
- Maintain accurate documentation, case notes, and client records.
Qualifications
Applicants must meet ONE of the following:
Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.
- Bachelor's degree with Social Service Worker (SSW) license
- Bachelor's degree in any field with willingness to pursue SSW licensure
Additional Requirements:
- Valid driver's license
- Basic understanding of Social Work principles
- Familiarity with social welfare policy and human development
- Ability to problem-solve and maintain a client-centered approach
- Spanish speakers may qualify for increased wages after testing

Employment at Wasatch Behavioral Health is contingent upon successful completion of a drug screen, motor vehicle record review and criminal background check.
