The Case Manager builds each participant's Individual Employment Plan, tracks progress through training and into work, and administers Barrier Reduction services so that practical obstacles do not end a participant's training. Essential duties and responsibilities

• Complete a needs assessment with each participant at enrollment and update it throughout the program.

• Develop the Individual Employment Plan with the participant within 10 business days of enrollment and amend it at each transition.

• Meet each participant weekly during training, track attendance and progress with instructors, and respond the same day to problems that interrupt training.

• Recommend Barrier Reduction services under the Hub's policy, such as transportation, child care, work clothing and emergency assistance, and keep the required documentation.

• Connect participants with community resources and coordinate referrals and handoffs with partner providers.

• Work with transition services staff on job readiness and placement, and provide retention support after placement.

• Carry out monthly follow-up contact for at least one year after completion.

• Record services, contacts and outcomes in the CEJA Reporting System within five business days.
