Location: Salt Lake City Area | Schedule: Typically, Monday - Friday 8:00 am – 5:00 pm | Type: Full-Time | FLSA: Non-Exempt

Utah Community Action is seeking a compassionate, organized, and resourceful Case Manager to help individuals and families achieve greater self-reliance. In this role, you will partner with clients to develop personalized case management plans focused on stable housing, employment, food security, financial literacy, and long-term success.

Key responsibilities include:

  • Provide comprehensive, client-centered case management and ongoing support.
  • Assess clients' social and financial needs, establish goals, and develop individualized action plans.
  • Connect clients with internal and community resources, including housing, employment, education, healthcare, mental health, childcare, transportation, and financial literacy services.
  • Maintain accurate and timely case documentation in the agency's electronic data system.
  • Monitor referrals, evaluate outcomes, and adjust service plans as needed.
  • Build collaborative relationships with community partners and represent Utah Community Action professionally.
  • Participate in outreach events, which may occasionally occur outside regular business hours.
  • Ensure compliance with all agency policies and funding requirements.

What You'll Need

Required Qualifications

  • Bachelor's degree in Social Work, Education, or a related field.
  • Two years of case management experience.
  • Proficiency with Microsoft Office.
  • Valid driver's license and ability to travel as needed.

Preferred Qualifications

  • Experience serving low-income populations.
  • Knowledge of trauma-informed care and conflict resolution.

Additional Requirements

Employment is contingent upon successfully passing a background check, a drug and alcohol screening, a sex offender registry check, a child abuse/neglect check, and meeting all Head Start health requirements.

As a federally funded organization, we are subject to applicable federal requirements. Although certain substances may be permitted under state law, federal requirements apply to employment with the organization.
