Case Manager – Newport News, VA Summary:Pyramid Healthcare at Newport News is seeking a dedicated Case Manager to support adults struggling with Substance Use Disorders. This role focuses on coordinating aftercare appointments, maintaining communication with referral sources, legal entities, and other agencies, and ensuring continuity of care throughout the treatment process. Responsibilities: Ensure clients have access to a full continuum of care by assessing needs and arranging appropriate services. Coordinate all aftercare plans, appointments, schedules, and meetings. Arrange halfway house interviews and placements when needed. Collect necessary information and releases from clients, families, referral sources, and other appropriate parties. Maintain accurate communication and documentation with external stakeholders regarding client treatment plans and progress. Perform additional duties as assigned. Required Skills & Experience: Bachelor’s degree in a related field required; Master’s degree preferred. Two years of previous experience in case management preferred. Schedule:Sunday – Thursday, 8:00 AM – 4:30 PM Compensation:$18.00 – $20.00 per hour, based on experience.
