Case Manager

RHD Delaware – Behavioral Health Home (BHH)

Position Summary

The Case Manager is responsible for assessing participant needs, coordinating services, implementing individualized service plans, and advocating for access to community resources that support recovery, stability, and overall well-being. This role works collaboratively with participants, families, providers, and community partners to ensure high-quality, person-centered care and successful participant outcomes.

Essential Duties and Responsibilities

Participant Assessment & Service Coordination

  • Assess participant needs and identify appropriate supports and resources.
  • Develop, implement, and monitor individualized service plans.
  • Coordinate care and services across providers, agencies, and community organizations.
  • Conduct screenings, referrals, and follow-up activities to ensure service engagement.
  • Support participants in achieving personal recovery, wellness, and treatment goals.

Advocacy & Resource Navigation

  • Advocate for participant access to healthcare, housing, employment, transportation, benefits, and other community resources.
  • Assist participants in obtaining and maintaining Medicaid, Social Security benefits, and other eligible services.
  • Educate participants and families regarding available resources and support systems.

Documentation & Compliance

  • Complete assessments, progress notes, service plans, and other required documentation accurately and timely.
  • Maintain participant records in accordance with agency, funder, licensing, and regulatory requirements.
  • Ensure compliance with confidentiality, HIPAA, and program standards.

Collaboration & Program Support

  • Participate in multidisciplinary team meetings and treatment planning activities.
  • Maintain productive relationships with community providers, referral sources, and stakeholders.
  • Identify service gaps and communicate recommendations for program improvements.
  • Support administrative and operational functions related to case management services.

Qualifications

Education

  • Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field required.

Experience

  • One (1) year of case management experience preferred.
  • Two (2) years of experience supporting individuals with behavioral health, mental health, substance use, or recovery-related needs preferred.

Additional Requirements

  • Valid driver's license and reliable transportation.
  • Ability to travel within the community to conduct participant visits and coordinate services.
  • Intermediate computer proficiency, including Microsoft Office applications and electronic record systems.

Work Environment & Physical Requirements

  • Work is performed in office, community, participant home, and provider settings.
  • Frequent travel throughout the service area is required.
  • Must be able to sit, stand, walk, drive, bend, reach, and climb stairs as needed.
  • Ability to lift up to 50 pounds occasionally.
  • Regular use of a computer, telephone, and other office equipment.
