Case Manager -

ESSENTIAL DUTIES
- Provide direct services to individuals and families in need of crisis intervention and eviction prevention services, including intake, engagement, assessment, service plan development, referrals, advocacy, exit summaries, and follow-up.
- Manage a caseload for both crisis intervention and eviction prevention services and connect clients to government benefits and entitlements that help them remain stably housed.
- Monitor and document client progress toward service plan goals in Salesforce Database.
- Document all interactions with, or on behalf of, clients in Salesforce database.
- Maintain familiarity with program resources available on- and off-site.
- Coordinate client services with other social services, mental health, medical, employment, educational, childcare, and other providers.
- Conduct regular case conferences with clients and service providers as required.
- Complete program reports monthly, or more frequently as required.
- Report to, and meet with, supervisor monthly.
- Conduct home visits, outreach events, and other fieldwork as required.
- Perform additional duties as assigned.
QUALIFICATIONS
- Bachelor’s Degree preferred/HS/GED with 1 year experience in benefits and gov’t entitlements
- Bilingual both verbal and written preferred
- Proficiency in modern business communications including personal computers, electronic mail, voicemail, and copier equipment
- Proficiency in virtual platforms is preferred
- Develop a customized service plan with clients, outlining action steps and responsibilities.
- Assist the client with obtaining necessary documentation for benefits and entitlements they want to acquire. This includes increasing the number of selected benefits, renewal of applications or the recertification process associated with the continuation of key benefits.
- Maintain up-to-date case-related applications and Salesforce database entries, as well as regular contact with clients through office visits, telephone contact, and home visits.
- Coordinate client referrals for other needed services
- Provide budget counseling to ensure effective money management skills.
- Other duties as assigned.
PHYSICAL REQUIREMENTS
- Ability to use a computer for prolonged periods
- Ability to occasionally lift and/or move up to 10 pounds
- Ability to stand, walk, or sit for long periods of time
- Ability to bend and retrieve objects and/or documents
- Ability to commute within the Bronx
- ‘TB Test required with the first 120 days of employment’ for Shelters and Homeless Outreach programs
