What You'll Do
As a Case Manager, you will:
- Coordinate medical care and treatment services for youth in care
- Maintain medical records and ensure compliance with regulatory requirements
- Schedule and coordinate routine, specialty, psychiatric, and follow-up appointments
- Assist with transportation to medical appointments as needed
- Monitor medication administration and complete weekly medication counts
- Train staff in medication administration, CPR, and First Aid procedures
- Communicate medical updates and treatment information with parents, guardians, providers, and treatment team members
- Collaborate closely with clinicians, direct care staff, families, and community partners to support positive outcomes
- Maintain accurate documentation and electronic records
- Assist with program operations and provide support coverage when necessary
- Support quality care initiatives and contribute to overall program success
- Perform additional duties as assigned

Qualifications
Required
- Bachelor's Degree and one year of related experience preferred, or an equivalent combination of education and experience
- Intermediate computer skills
- Ability to establish a HIPAA-compliant home office
- Valid driver's license
- Ability to pass drug screening and state and federal background checks
Preferred
- Previous experience in behavioral health, human services, case management, or residential care settings
- Experience working with adolescents and families
Schedule
- Part-Time
- Friday: 12:00 AM to 8:00 AM
- Saturday: 12:00 AM to 8:00 AM
- Additional weekday hours may be assigned to meet full-time status
- Occasional flexibility may be required based on program needs
Physical Requirements
- Ability to lift up to 25 pounds
- Ability to stand, walk, bend, and assist youth as needed
- Ability to perform essential job functions with or without reasonable accommodation
