Responsibilities:
- Conduct comprehensive client assessments to identify needs, strengths, and barriers to stability
- Develop and implement individualized service plans in collaboration with clients
- Connect clients with community resources, social services, and support programs
- Monitor client progress and adjust care plans as needed to ensure goal achievement
- Maintain accurate and up-to-date case documentation and client records
- Advocate on behalf of clients with external agencies, providers, and stakeholders
- Facilitate referrals to mental health, housing, employment, and other support services
Requirements:
- Bachelor's degree in Social Work, Human Services, Psychology, or a related field
- Prior experience in case management, social services, or community outreach preferred
- Strong knowledge of local New Haven and Connecticut community resources
- Excellent communication, advocacy, and interpersonal skills
- Ability to manage a caseload effectively while maintaining attention to detail
- Empathetic, culturally competent, and trauma-informed approach to client care
- Proficiency in case management software and Microsoft Office Suite
