This position reports to, is supervised by, and evaluated by the Associate Vice President of Student Support. The evaluation of work performance will be ongoing and will be carried out by the Associate Vice President of Student Support and will include the specific duties and responsibilities of this position description plus employee attitude and general working behavior.

Job Purpose: The Case Manager is responsible for developing the mission, vision and daily operations of case management while supporting the administration and management of all case management including programs, budgets, services, marketing, and assessment. This position plays a vital role in supporting a mission-based staff environment that values collaboration, teamwork, well-being, and a student-centered approach.  

This position will work collaboratively to create, enhance, support, and foster initiatives designed to promote holistic student well-being by engaging students in opportunities to reflect on and advocate for their personal well-being; providing support services and resources for students struggling or in crisis; and ensuring the campus community is aware of the resources available to support students of concern. This position is responsible for researching best practices and finding ways to improve or implement services that align with the identified needs of the University of Lynchburg

Specific Job Duties:


Develop and implement Case Management core values, strategic plan, and purpose statement in alignment with the University of Lynchburg’s Mission Statement and Strategic Plan, the Student Support mission and goals, and the needs of the Lynchburg community.
Administer the day-to-day management of Case Management, including handling multiple responsibilities and duties simultaneously, frequently in a fast-paced environment; including:

overseeing case management response to and support of critical student concerns
administering operating practices/policies,
being available for frequent consultation and support on cases, 
crisis triage and management procedures,
administering, creating and facilitating record keeping and communication processes in accordance with best practice and legal standards,
staying current on current case law/legal issues that may impact case management work,
frequently taking the lead on communication with multiple stakeholders including, students, staff, faculty, parents, and external partners (local crisis counselors and mental health professionals, medical staff, and more) in the face of high-pressure circumstances.


Coordinate the Medical Leave of Absence process and the General Leave of Absence Process.
Evaluation and continuous development of the case management system through evaluation of data, research of best practices, development of procedures and manuals, training and presentations, communication and record keeping processes, assessment/evaluation of services, and creating and implementing new initiatives, programs and services.
Co-Chair the University’s Campus Assessment Response and Education (CARE) Team, including responsibility for any training, processes and/or publicity about the team.
Coordinates and attends meetings related to specific students of concern, including participation in the Behavioral Intervention Team and Threat Assessment Team, as requested.
Facilitates the Student of Concern Referral form, or similar critical well-being alert system, for the University and coordinates response to student concerns including all processes for receiving, triaging, responding to and assigning cases from forms, coordinating follow up efforts for referrals and referrers, and data analysis of form information.
Support the On-Call team which is available 24 hours a day, when regular classes are in session (if operational).
Collaborate with all relevant University and community partners including local mental health agencies and medical providers to maintain knowledge of current procedures to assist students in navigating to the appropriate resources.
Keep an up to date list of community resources available to students.
Lead on-going assessment of all learning and program outcomes within the case management area; employ evidence-based decision making in evaluating and fine-tuning existing programs.
Attend weekly on call debriefing meetings to help determine students needing follow up and attention.
Advise on creation, training, and implementation of appropriate University emergency and crisis response protocols.
Manage and is accountable for an operating budget associated with this position specific to personnel and programmatic needs.
Perform other duties as assigned.

QualificationsExperience/Skills:


Excellent communication skills in person, small or large groups; effective public speaking skills and the ability to interact with a wide-range of diverse constituents. Excellent written language skills.


Physical Demands:


Ability to sit for extended periods of time
Ability to move about campus as needed to deliver items, attend meetings, etc.
Extensive wrist and hand movements as related to keyboarding
Ability to bend, kneel, stop and reach as needed for routine office duties
Ability to work late night and weekend hours as needed


 

Required Qualifications:


Experience in a Higher Education environment or relevant field of experience.


Education: 


Bachelor's degree in Social Work, Counseling, Higher Education Administration, or other related field. 


Preferred Education: 


Master’s degree in Social Work, Counseling, Higher Education Administration, or other related field.
