JOB SUMMARY: The Case Manager plays a critical role in supporting patients throughout their recovery journey by coordinating services, reducing barriers to care, and facilitating successful transitions into the community. This position focuses on connecting patients with community-based resources that promote long-term stability, independence, and sustained recovery while participating in treatment and following discharge.

Working collaboratively with the interdisciplinary treatment team, the Case Manager assists patients in developing individualized discharge and aftercare plans, securing essential resources, and building the skills and support systems necessary for independent living.

CASE MANAGER JOB DUTIES:

  • Assists with comprehensive psychosocial and resource assessments to identify patient needs, strengths, and barriers to successful recovery.
  • Develop individualized case management and discharge plans in collaboration with patients and the clinical treatment team.
  • Connect patients with community resources that support long-term recovery and independence, including:
    • Housing and sober living programs
    • Employment and vocational rehabilitation services
    • Educational and workforce development opportunities
    • Transportation assistance
    • Health insurance and benefits enrollment
    • Primary care and specialty medical providers
    • Mental health providers
    • Peer recovery support services
    • Recovery community organizations
    • Food assistance and other public benefits
    • Legal aid and court-related resources
    • Financial assistance programs
    • Childcare and family support services, when applicable
  • Coordinate referrals and assist patients in completing applications, scheduling appointments, and obtaining required documentation.
  • Collaborate with probation officers, social service agencies, healthcare providers, employers, educational institutions, and other community partners as appropriate.
  • Educate patients on available community resources and empower them to independently access and navigate support systems.
  • Monitor progress toward case management goals and modify service plans as patient needs evolve.
  • Participate in multidisciplinary treatment team meetings and contribute to treatment planning.
  • Maintain timely, accurate, and compliant documentation within the electronic health record.
  • Ensure all case management activities comply with federal, state, accreditation, and organizational standards.
  • Maintain current knowledge of local community resources, recovery support services, and public assistance programs.
  • Assist patients with discharge planning beginning early in treatment to promote continuity of care and reduce risk of relapse or readmission.
  • Advocate for patients by addressing barriers to housing, employment, healthcare access, transportation, and other social determinants of health.
  • Participate in quality improvement initiatives and community outreach efforts if required.
  • Other duties as assigned.

MINIMUM QUALIFICATIONS: 

  • Bachelor’s Degree (Master’s preferred) from an accredited college or university in social work, human services, psychology, counseling, or a related field
  • Minimum of one year of experience in case management, behavioral health, substance use treatment, social services, or a related field.
  • Knowledge of substance use disorders, recovery principles, and community-based support services.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to establish rapport with individuals from diverse backgrounds.
  • Proficiency with computers and Microsoft Office applications.
  • Valid driver's license and acceptable driving record.
