Position Overview

 

As a Case Manager with HELP USA, you’ll provide case management and crisis intervention services to residents in one of our supportive permanent housing programs with a goal of promoting independent living skills, increased housing stability, and well-being. HELP One Building C provides supportive services to 48 formerly homeless single adults.

 

Duties/Responsibilities

 

  • Provide initial/annual assessments for single adult residents, provide service plan development, case management, supported referrals, and on-going support to assist tenants to develop skills and achieve personal goals associated with housing stability and well-being.  Document all services delivered with and on behalf of the tenants in AWARDS.
  • Ensure all data collected in AWARDS is accurate and entered in a timely manner.
  • Serve as a liaison and advocate with medical, mental health and substance abuse providers.
  • Develops linkages with community programs to support the identified needs of tenants including, but not limited to, education/training, employment, mental health substance abuse, medical, rental assistance, financial issues, etc.
  • Assist tenants overcome barriers that may affect the timely submission of monthly rent.
  • Work with property management to support tenants in all aspects of maintaining permanent housing.
  • Escort residents to appointments as needed (i.e. HRA, HPD, etc.)

 

Education/Qualifications

 

  • Bachelor’s Degree in Social Work, Counseling, Psychology, Human Services, or a related field required.  Minimum of seven (7) years’ experience in delivering intensive case management.
  • Experience in working with homeless populations, especially single adults with SMI (Severe Mental Illness), SUD (Substance Use Disorder) and Housing First model preferred.
  • Knowledge of AWADS a plus.
  • Computer literate specifically with Microsoft applications required.
  • Bilingual a plus.

Compensation

Actual compensation will not be less than the applicable minimum wage, prevailing wage, or exempt salary requirements under federal, state and local laws.
