Position Summary
The behavioral health Case Manager is a core member of the collaborative care team that includes therapists, psychiatrists, and ancillary support staff. The Case Manager's overall responsibility is to support client care.

Relationships and Contacts

Within the organization: Maintains frequent and close working relationships with the Executive Director, Business Development team members, Admissions team members, Nursing leadership, milieu leadership, and all clinical team members.

Outside the organization: Initiates and maintains strong professional relationships with clients and families, clinical providers, referral partners and vendors, as needed.

Essential Responsibilities
   1. Supports the needs of clients throughout the treatment process, to include administrative support.
   2. Supports the admissions process, to include client orientation.
   3. Support business office functions related to payment collections and records maintenance.
   4. Greets clients at the beginning of the day, running morning intentions group with clients.
   5. Provides a high level of customer service to clients, families and clinical partners throughout the
        treatment process.
   6. Follows up with all clients after leaving treatment on a regular basis, ensuring access to needed resources are offered.
   7. Manage issues related to FMLA, EAP, Disability, Legal, Financial, Personal Therapy, etc.
   8. Coordinates communication with clinical partners in facilitation of admissions process and ongoing client care.
   9. Provides group facilitation, as needed and directed by clinical leadership.
  10. Supports clinical team with group/schedule coordination, discharge planning, and records maintenance.
  11. Supports client communication and feedback initiatives, to include satisfaction surveys and Google Reviews.
  12. Help schedule medical and provider appointments while in treatment.
  13. Supports and participates in interdisciplinary meetings.
  14. Coordinates support groups as needed.

Additional Responsibilities


Functions within the guidelines of the facility’s Code of Ethics and in accordance with corporate compliance.
Conducts chart audits following program policy timelines, as requested.
Consistently arrives to work on time, rarely missing scheduled shifts, and promptly notifies supervisor when absent.
Demonstrates an ability to adapt to changes in the facility function, management styles, and facility routines.
Reads, understands, adheres to, and models all company policy statements on ethics, conduct, and conflict of interests.
Adheres to facility policies, procedures, rules, and regulations, implementing regularly.
Maintains positive attitude and acts as a team player with others on the treatment team.

QualificationsRequirements
 

Education and Experience
Position requires a high school diploma with a minimum of 2+ years’ experience in a behavioral healthcare setting working with adults suffering with mental health issues. Bachelor's degree preferred.

 

Skills and Competencies


 Knowledge and experience with maintaining and securing medical records including HIPAA, Quality
Assurance reviews and ROI consents.
 Strong customer service orientation required and experience successfully resolving client and/or family questions or concerns.
Experience maintaining accurate reports related to census, time and attendance, etc.
Comfort with computer software applications including Outlook, Word, and use of standard office equipment. Advanced knowledge of Excel preferred.
Performs duties independently, responsibly and with a high level of integrity.
Excellent verbal and documentation skills
Ability to participate in Treatment Planning
Ability to perform needs assessments
Excellent organizational skills
