Job Position – Case Manager 

(Position Description - full-time, hourly, non-exempt, and at-will)

Location: Crossings Community Clinic – Penn


General Scope of Position:

The Case Manager is a member of the clinic care team and functions as an advocate and community resource navigator for our patients. This involves assessing, identifying, and developing actionable plans to address social determinants of health and provide care coordination services. In addition, the Case Manager will cultivate and maintain relationships with community partners/organizations. The Case Manager will also work closely with the Medical Director and Clinic Operations Manager in coordinating patient care. This position is a collaborative member of our health care team performing duties and responsibilities in a manner consistent with the Christian mission and values of Crossings Community Clinic. (CCC or Clinic).


Typical Clinic hours are Monday thru Friday 8 am - 5 pm but actual days and hours of work are subject to Clinic needs or special events, which may require schedule flexibility including days and hours beyond or different from a typical work week.


This position is a full-time, at-will hourly employment position; occasional evenings/weekends.


Organizational Relationships:

Reports to Crossings Community Clinic Operations Manager and will work collaboratively and interact regularly with various vendors, CCC staff and volunteers across the various departments and ministries, and locations, as well with partners of the Clinic to accomplish the goals and mission of the Clinic. Qualifications: 

  • Bilingual in English/Spanish preferred. 

  • 1+ year experience in case management/social work.

  • Bachelor’s degree in social work or psychology.

  • Preferred active licensed clinical social worker with the Oklahoma Board.

  • Heart of compassion and is a confidential steward of patient privacy.

  • Is willing and able to pray with patients and team members.

  • Is willing and able to provide patient spiritual care.

  • Intermediate computer skills – will use various applications including Epic and Volgistics.

  • Ability to work effectively as a ministry team member ensuring excellent patient care.

  • Ability to be a spiritual leader among the staff and volunteers with a strong commitment to the Clinic mission.

  • Exhibits strong spiritual gifts of discernment, compassion, teaching and encouragement.

  • Professional verbal and written communication.

  • Assist in maintaining the highest possible clinic standards and professional ethics in accordance with the goals, Purpose Statement, and Core Values of Crossings Community Clinic.

  • Support the leadership of CCC, contributing to the spirit of unity among the staff and patients.

  • Represent the Clinic in civic and clinic-related efforts as approved.


Responsibilities

  • Engages in basic generalist practice that includes interviewing, assessment, planning, intervention, evaluation, case management, information and referral, problem solving, consultation, supportive counseling, education, advocacy and community organization.

  • Develops an action/case plan with goals, based on functioning level, physical/medical condition, support network, psychosocial and financial status based on long and short-term needs.

  • Develops and maintains effective working relationships with individual patients and/or family.

  • Serves as a source of social and spiritual support for patients and/or families.

  • Makes referrals to community agencies and conducts follow-up activities.

  • Serves as patient liaison and advocate.

  • Provides transitional care services to patients who acquire health insurance and are no longer eligible to receive care at Crossings Community Clinic.

  • Develops and maintains a cumulative record for each patient receiving case management services as required by professional licensure standards and Crossings Community Clinic policies and procedures.

  • Works as part of an interdisciplinary team to provide integrated services.

  • Communicates effectively with patients, Crossings Community Clinic staff, and participants in an interdisciplinary team.

  • Cooperates with and contributes to the development and implementation of an interdisciplinary treatment plan.

  • Promotes interactive communication and develops and refines linkages between Crossings Community Clinic/Center programs and community health care facilities, social service providers, schools, and other agencies.

  • Cooperates with local medical, mental health and community agencies

  • Maintains documentation of community resources.

  • Lead the continued development of the case management program and services.

  • Participates in activities to develop and maintain quality assurance indicators that evaluate the effectiveness of the case management program.

  • Implements Crossings Community Clinic policies and procedures and recommends any needed modifications.

  • Participates in community events during business work week, weekends and evenings as needed.

  • Sustains productivity expectations established by the supervisor.

  • Performs other duties as assigned.

  • Mental Health Case Manager needed in Oklahoma City, Oklahoma to serve adults ages 18-64 as a collaborative member of the clinical care team, provide advocacy, coordinate care, navigate community resources, develop case plans, maintain patient relationships, support interdisciplinary care, and lead the growth of the case management program in alignment with the mission of the clinic.






Special Knowledge, Skills and Abilities:

  • Bilingual in English/Spanish preferred.


Physical and Other Requirements:

  • Ability to meet the physical and work environmental demands of the job, and perform all essential job functions.

  • Be able to stand for up to two hours, walk a quarter of a mile, and to lift and carry 15-25 pounds for 500 feet.

  • Possess a current unrestricted driver’s license.

  • Ability to travel between locations as needed or requested.

  • Has the ability for a flexible schedule as some evenings and weekend hours may be required.

  • Maintain a vibrant relationship with Christ through prayer, Bible study, regular worship attendance and Christian fellowship.
