Position Summary: The Case Manager participates as a member of the Jail Based program team to intensively support patients by initiating services while incarcerated and to continue services into the community. This position is part of a multidisciplinary team that provides services designed to assist and support an individual diagnosed with or being assessed for a mental health disorder, to gain access to needed medical, social, educational, and other services as well as provide care coordination and care transition services.
Qualifications: Our ideal candidate for our Case Manager position will maintain a bachelor’s degree in health or human service-related field. Additionally, they will have a minimum of 1 year in health or human services setting; Preferred 2 years case management experience. Additional Certifications: CAT or CAS
Salary: Starting at $18.50 - $21.72 per hour (non-exempt/hourly) and considers preferred experience and preferred education described above.

Working Conditions/Physical Demands:
Employees must occasionally lift and/or move 15 to 50 pounds. Employees are frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for long periods of time at computer.
Employees must be able to travel between Axis locations. Depending on the position, employee may also be expected to engage in occasional statewide and domestic travel.
Working in a healthcare organization lends itself to the possibility of exposure to communicable disease.
Noise level in the working environment is usually moderate.
Annual Flu immunization by November 1, annual TB Screening, complete de-escalation training, and BLS certification, as required for position.

Knowledge and ability to operate standard office equipment including scanner, computer, calculator, copy machine, fax and multi-line telephone.

Ability to be helpful and sensitive to the needs of all employees, while concurrently respecting corporate policies, procedures and priorities.
Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
Ability to communicate job-related information to individuals and small groups.
Ability to work independently with minimal supervision; plan own work schedule effectively and multi-task and prioritize in a fast-paced setting.
Ability to multi-task, organize and prioritize workload in a sometimes-hectic environment with frequent interruptions.
Ability to concentrate in sometimes hectic and stressful settings.
Ability to read and understand written materials.
Must regard work as confidential.
Must have current driver’s license, qualify for commercial vehicle insurance and maintain a driving record that allows insurability with Axis Health System’ insurance agency.
