Case Management Intensive Care Coordination Supervisor

Annual Salary: $57,119

Work Schedule: Monday - Friday 8:30 a.m. - 5:00 p.m., with on-call responsibilities

The Case Management Supervisor (Intensive Care Coordination) provides day-to-day oversight of intensive care coordination and case management services for children, adolescents, adults, and families impacted by serious emotional disturbance, mental illness, intellectual/developmental disabilities, and substance use disorders. This position ensures services are timely, compliant, and clinically appropriate while strengthening staff performance and program results.

Key responsibilities include supervising approximately 6–8 Case Managers; guiding clinical assessments, service planning, utilization management, and crisis support; coordinating hospital liaison and discharge planning; assigning and balancing team caseloads; training and onboarding staff; partnering with community stakeholders (including courts, law enforcement, and health providers); monitoring unit expenditures; and recommending program/policy improvements.

Required qualifications/skills:
- Bachelor’s degree in Human Services such as Social Work, Psychology, Sociology, or Counseling.
- Three (3) years of experience providing case management services to the assigned client population.
