Position Title: Case Management/ Recovery Support

Reports To: Coordinator of Case Management

Job Type: Full-Time

Schedule:  M-F, 9 am to 5 pm, 1 Saturday per month from 9:00 a.m. to 4:00 p.m.


The Link & Option Center, Inc. is a Joint Commission–accredited outpatient State Certified Community Behavioral Health Clinic (CCBHC) serving the far Southside community and designated to serve the Southland region with locations in South Holland and Hazel Crest, Illinois. Founded in 1994, The Link & Option Center, Inc., provides comprehensive behavioral health services including assessment, psychiatry, therapy, crisis services, case management, and care coordination.

Responsibilities:

The Case Manager (CM) is responsible for ensuring continuity of care for TLOC clients who are identified as high-risk for psychiatric hospitalization or detainment and those who are at risk of being Lost-to-care. The CM ensures that clients are monitored consistently for clinical and/or medical engagement, and that their Social Drivers of Health (SDOH) needs are met to ensure that clients remain stable within the community and minimally utilize institutional care. The CM provides close and consistent tracking of TLOC clients’ engagement in primary care, psychiatry, and other needed services. The CM is the primary point of contact for identified clients and provides overall management of the system of care and client engagement within the system (i.e., reducing barriers to engagement such as transportation, lack of social supports, and access to needed community resources. 

Specific duties include:

    Completes Individual Treatment Plan (ITP).

    Conducts well-being checks on clients who demonstrate risk factors related to mental and physical illness.

    Maintains at least monthly contact with assigned clients.

    Provides community support to individuals and groups as needed.

    Collaborates with internal and external service providers (i.e. psychiatry, primary care, therapists, etc.) to ensure continuity of care.

    Aids clients in securing public entitlements (i.e., public aid, SSI, etc.)

    Responds to crises in a timely manner.

    Documents client engagement in the TLOC EHR system in accordance with agency policy and procedures.

    Collaborates with clients to complete a Wellness Recovery Action Plan (WRAP).

    Monitors client progress toward achieving ITP and WRAP goals and objectives.

    Track client engagement in treatment and conduct follow up on clients who are poorly engaged.

    Completes billing documentation in accordance with TLOC Policy and Procedures.

    Participates in weekly supervision.

Qualifications and education requirements

    Bachelor’s degree in health care administration, business administration, or a related human services field.

    Demonstrated knowledge of Case Management best practices. JCAHO accreditation standards.

    Demonstrated knowledge of IL Administrative Rules 132 Reimbursement Guide.

    High degree of professionalism in dealing with diverse groups of people, including clients, staff, and others.

    Demonstrated ability to work collaboratively with internal and external providers.

    Demonstrated ability to accept constructive criticism and feedback from peers and supervisor.

    Demonstrated effective verbal and written communication skills.

    Must be culturally competent, empathetic, and able to interact and develop rapport with a diversified population.
