GENERAL SUMMARY:  

Reporting to the Director of Client Services, the Care Management Supervisor is responsible for the administrative and clinical supervision and support to assigned staff that assesses needs, coordinate services and monitor services enabling disabled and older adults to live at home. The Supervisor trains assigned Care Managers, approves and monitors care plans to ensure consumer’s needs are met, are cost effective and are provided in compliance with regulations.  The Care Management Supervisor reviews department policies and procedures and makes suggestions for improvement. Working with all Agency departments and staff, the Care Management Supervisor engages in quality assurance programs and the implementation of improvement strategies. The Care Management Supervisor is a member of the Leadership Team of the agency.

ESSENTIAL FUNCTIONS:


Provide leadership to a team of Care Managers including plan, direct, assign, review and evaluate the work of assigned Care Managers on an ongoing basis and provide regular individualized and group supervision, guidance and professional development. This includes:


Review work for timeliness, completeness, quality; approve and monitor service plans including utilizing Wellsky and Quality Assurance reports.
Orient and conduct regular training for Care Managers on casework methods to ensure each possess the required competencies to successfully perform their job.
Conduct meetings with Care Managers on an individual basis to discuss specific case problems and work performance issues; provides written and verbal evaluations on an ongoing basis; counsels and if needed, disciplines employees on work performance issues when necessary; seeks advice from Director of Client Services and/or Human Resources.
Accompany Care Managers on visits when needed.
Participate in the recruitment of new staff by interviewing applicants and providing input on hiring.





Ensure Executive Office of Aging and Independence (AGE) programs and mandates are implemented including timelines for visits and program eligibility.
Provide training on AGE forms, use of the Wellsky for Care Management staff.
Develop and implement Care Management programs.
Provide coverage for Care Managers when needed including assisting with home visits, discharge planning functions and coordination of service.
Work with the Director of Client Services and Quality Assurance & Compliance Analyst to provide and evaluate data and statistics for planning, development, and quality assurance purposes.
Work collaboratively with all Minuteman Departments and Staff, community providers, other ASAP’s, and AGE.
Participate in new or existing Agency or statewide committees or meetings as requested by the Director of Client Services.
Create a positive working environment conducive to supporting the work of the Care Management and other agency staff.

WORK ENVIRONMENT:

Most of the time is spent in a professional environment.  This role regularly requires the use of a computer, telephone and other standard office equipment. The employee will travel by car to meet with consumers where they live. The CMS works in varied residential environments, and may be required to stand for long periods of time, navigate potentially unsafe environments, and navigate stairs to enter consumer homes.

QualificationsQUALIFIED APPLICANTS WILL HAVE:


Bachelor’s degree in social work or related field and supervisory experience in the human services field required.
Minimum of two years in the human service field.
Knowledge of older adult service delivery systems, AGE programs, Medicaid Long-Term Care Programs, and ASAP Programs preferred.
Experience working in a non-profit environment with older or disabled adults desired.
Ability to be flexible and creative in problem solving
Experience in computer systems; an ability to teach these skills to other staff.
Superior leadership traits that motivate, influence, develop, and encourage staff collaboration.
