Care Manager - Middlesex County - Part time - Hourly rate $43.00 - The areas highest quality behavioral healthcare providing care management services to clients.  Responsibilities include assessing and assisting with clients needs and coordinating care in a warm and compassionate environment, 8 +/- hours per week, 9:00am - 5:00pm Monday - Friday - The Care Manager will follow through and assist clients with their plan of care, and coordinate care services ensuring that those we serve receive the highest quality of service in a caring and compassionate atmosphere.

MINIMUM QUALIFICATIONS 

  • Bachelor's degree required
  • Ability to read, write, understand, and communicate effectively in English
  • Drivers license in good standing
  • Possess insured vehicle to travel to and from clients homes
  • Ability to deal with others in a diplomatic, tactful and confidential manner 
  • Ability to maintain composure under stress in dealing with residents, staff, and families
  • Flu vaccine required during flu season

Senior Care Options by Parker:

Senior Care Options by Parker provides customized Geriatric Care Management services as well as Behavioral Health services by licensed social workers to individuals in their homes and communities across Middlesex, Monmouth, and Ocean Counties. Program participants of both programs benefit from services offered in their own environment by our dedicated care partners who are committed to meeting their needs. Services will help support  participants in maintaining independence in their own environment.
