This role requires frequent travel. You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).

 

Key Responsibilities

  • Conduct home visits to implement care plans with clients and ensure a continued high level of care.

  • Train and support caregivers in meeting their loved ones' needs.

  • Monitor and report on clients’ health, coordinating with their care team and outside agencies as needed.

  • Facilitate smooth transitions and ongoing care for clients.

  • Maintain compliance with all relevant regulations and standards.

  • Full-time hybrid position offering autonomy while utilizing your assessment, care planning and case management skills.

 

What You Will Bring (Requirements)

  • Bachelor’s degree required; preferably in social work, human services, or a related field.

    • LSW or LISW in state of employment preferred.

  • At least one (1) year of experience working with elders or adults with disabilities.

  • Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.

  • Experience in case management, care implementation, planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.

  • Training, evaluating, and supervising caregivers.

  • Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting their care related needs.

  • Documenting in an accurate and timely manner; maintain a clinical record for each  patient.

  • Proficiency in common software and web-based applications (e.g. MS Office, case management systems, EVV, etc.).

  • Patient-centered approach with strong communication skills.

  • Familiarity with New Hampshire Medicaid, CFI and/or caregiving ecosystem (including CMAs, government offices, and other stakeholders) is preferred.
