The Autism Society of North Carolina (ASNC) is growing our Tailored Care Management team! We are a high-quality Care Management agency, with one of the highest engagement rates in the state. As a niche program, we are able to offer Care Managers and families a truly individualized experience that is focused on quality. This is a hybrid position that requires travel around the Triangle Area (Wake, Durham, Orange Counties), NC.
What You’ll Do:
Provide whole-person care management for children with autism or other developmental disabilities
Collaborate with care teams, service providers, and families
Work with families to identify needs and provide resources and support to meet those needs
Submit referrals, manage authorizations, and monitor service delivery
What We’re Looking For:
Bachelor’s or master's in human services (Social Work, Psychology, etc.)
2+ years' experience with Autism and/or Intellectual /Developmental Disabilities
2+ years’ experience providing care management, case management, or coordination to individuals with IDD/ASD
Based in or near the Triangle area and willing to travel locally
Strong communication, organization, and problem-solving skills
Self-starter who thrives in a collaborative environment
