About the Role
Liberty Resources is seeking an Adult Health Home Care Manager to support individuals with complex medical, behavioral health, and social service needs throughout Madison County. This role helps adults access essential services, improve health outcomes, and navigate community resources through comprehensive care coordination.
This is a full-time, hybrid position requiring travel throughout the community to conduct home and community-based visits.
Care Manager Key Responsibilities
- Assess eligibility and enroll individuals in Health Home Care Management services.
- Complete comprehensive assessments and develop person-centered care plans.
- Coordinate care with healthcare providers, behavioral health professionals, and community partners.
- Connect individuals to medical, mental health, substance use, housing, and social service resources.
- Conduct home and community visits to engage and support members.
- Manage a caseload of approximately 30 clients.
- Participate in an on-call rotation for after-hours support and crisis response.
Care Manager Qualifications
- Bachelor's degree with 2 years of relevant health or human services experience, or
- Master's degree with 1 year of relevant experience, or
- Registered Nurse with 2 years of relevant experience.
- Experience with care coordination, case management, or serving adults with serious mental illness, developmental disabilities, substance use disorders, or chronic medical conditions preferred.
- Valid New York State driver's license and reliable transportation required.
