Screening and Assessment

Conducts interviews with individuals (and legal guardians as indicated) to collect information both initially and ongoing.
Takes and records vital signs, administers injections, checks, reads, and records blood sugar, draws blood, and collects urine drug screens as ordered by physician/practitioner.


Coordination of Services

Collaborates with individuals, families, physicians, health care providers, and team members.
Participates in comprehensive care plan meetings and transitional care planning.
Schedules appointments and ensures transportation for individuals served.
Prepares and maintains case records within program guidelines.


Provision of Services

Provides medication reconciliation and education.
Educates served regarding a variety of health issues to ensure understanding of care plan and needed action.
Encourages individuals to remain in a continuum of care to ensure success.


QualificationsPrimary Attributes
Professional & Technical Knowledge:

Possesses the equivalent of formal training in nursing, usually in the form of a major in Registered Nursing as part of an associate’s degree, bachelor’s degree, or master’s degree Program.


Continuing Education:


Maintains current knowledge of standards of care and practices, typically acquired through continuing education.


Licenses & Certifications:


A valid, current Registered Nursing License in the state of West Virginia as well as a valid state-issued driver’s license is required.


Technical Skills:


Prepares basic correspondence and simple reports in Microsoft Word. Uses Microsoft Excel to create tables and simple displays of information. Creates basic presentations in Microsoft PowerPoint. Demonstrates necessary proficiency with all electronic clinical systems, including EHR and scheduling systems, in use at the Company.


General Development:


Displays basic organizational skills in order to self-prioritize and organize own work. Determines effective responses to situations encountered within established precedent. Able to effectively communicate opinions drawn from conclusions using inference and logic. Exercises tact and diplomacy in the resolution of mild conflicts or disagreements.


Work Environment:


Work is performed in a public, customer-facing, high traffic environment. At times, work may involve working with disruptive or distressed individuals.
