Care Coordinator Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Adult Services


Role Responsibilities

The Care Coordinator TCOOMMI provides direct services to individuals diagnosed with chronic, serious mental illness. Responsibilities include assessing, planning, coordinating, monitoring, and linking patients to community resources and service providers. This position also provides psychosocial rehab and crisis intervention services while working in a community-based setting. This position also coordinates services with Community Supervision including probation and parole. Other key responsibilities include conducting needs assessments, recovery planning, and completion of documentation. 

The Care Coordinator TCOOMMI performs other duties as assigned by the supervisor. Must be able to work within a multi-disciplinary treatment team. This position requires excellent client engagement, time management, organizational skills and the ability to work within a multidisciplinary environment. Routine hours are 8:00 AM to 5 PM, Monday through Friday with flexibility for schedule changes to allow for appointments outside of routine hours. Must have reliable vehicle for transportation with paid mileage paid. 

The Care Coordinator TCOOMMI reports directly to the TCOOMMI Team Leader and is responsible and accountable for the adherence to and promotion of compliance policy. 

Salary

  • 90% $23.02 hour to start
  • 95% $24.30 hour at 6 mos. with satisfactory performance
  • 100% $25.58 hour at 1 year with satisfactory performance 

Required Education and/or Certifications, Experience, Skills

  • Bachelor’s or advanced degree from an accredited college or University. Counseling, educational psychology, or human

service field, early childhood intervention, gerontology, human growth and development, medicine, nursing, physician assistant. Psychology, rehabilitation, social work, sociology and special education.


  • Texas driver’s license in good standing as defined by Texana Center’s procedure.


  • 1 year of work experience providing direct services within a Behavioral Health or Social Service setting.

Preferred Education and/or Certifications, Experience, Skills

  • 3 years of work experience providing direct services within a Behavioral Health or Social Service setting.
  • Work experience providing direct services to individuals involved in the criminal justice system (Probation or Parole).
  • Spanish speaking.

Job Related Physical Requirements

  • Must be able to perform SAMA and CPR. 
  • Must be able to perform moderate lifting and carrying (15-44#), pushing, stooping, repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting. 
  • Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception. 
  • Qualified individual must be able to perform these functions with or without reasonable accommodations.
