Position Purpose

The Care Coordinator is responsible for coordinating services, support, and resources to ensure individuals receive high‑quality, person‑centered care. This role serves as a key liaison among individuals, internal teams, and external providers to promote continuity of care, safety, and positive outcomes.

What will you do:

  • Serve as the primary point of contact for care-related communication and problem-solving
  • Facilitate referrals and connections to community resources, medical providers, and support services
  • Monitor service delivery, identify gaps, and recommend adjustments as needed
  • Coordinate resident admissions, orientations, and discharges in compliance with state, county, and organizational requirements.
  • Advocate for individuals to ensure their needs, preferences, and rights are respected
  • Respond to concerns or incidents and support resolution according to policy
  • Collaborate with supervisors and leadership to support program goals and outcomes

Minimum Qualifications

  • Bachelor’s degree and 3 years of related experience OR associate’s degree and 4 years of related experience OR a High School Diploma and 9 years of related experience.
  • Valid Driver’s license and Ability to operate an organizational vehicle
  • Act 33/34/73 clearances
  • Pre-employment drug screen
  • Knowledge of community resources, service systems, and care planning processes
  • Strong organizational, communication, and problem-solving skills
  • Ability to work independently and manage multiple priorities
  • Proficiency with documentation systems and basic computer applications

POSITION HIGHLIGHTS/BENEFITS:

  • WORK SCHEDULE: This is a Non-Exempt (hourly) position.  Primarily Monday through Friday daylight (8am-4pm) with flexibility to meet resident needs; some evenings, weekends, and holidays may be required.
