Geode Health is a rapidly growing, national provider of outpatient mental health services. Our Mission is to “Transform mental health by making it more accessible, affordable and effective”.

We are seeking a highly motivated and empathetic individual to join our team as a Mental Health Care Coordinator. The primary responsibility of this role is to launch a new clinical program that supports high-risk patients. The successful candidate will work closely with healthcare providers to coordinate and facilitate the implementation of individualized care plans for patients with high symptom severity.

This is a fully remote role, overseeing the Care Coordination for our offices in Texas.

Key Responsibilities:

  • Reach out to high-risk patients, as flagged by mental health providers, to facilitate the implementation of their personalized care plans, ensuring understanding and commitment.
  • Act as a liaison between patients and healthcare providers to ensure seamless communication and coordination throughout the care process.
  • Manage and organize patient lists, ensuring timely follow-ups and tracking progress.
  • Utilize technology for analyzing patient data and streamlining the care coordination process.
  • Provide ongoing support and guidance to patients as they navigate their care plans.
  • Collaborate with cross-functional teams, including providers, operations, and support staff, to enhance overall patient care.

Qualifications:

  • 5+ years of experience in Care Coordination, Navigation, or Social Work, preferably within a healthcare setting.
  • Demonstrated empathy and strong interpersonal skills, with the ability to connect with patients and understand their unique needs.
  • Excellent organizational and process-oriented skills, including the ability to manage lists and ensure follow-up.
  • Comfortable with technology and electronic health records.
  • Ability to work collaboratively and cross-functionally with various departments.
  • Clinical background is a plus but not required.
  • Strong communication skills, both written and verbal.
