Care Coordinator / Case Manager

Graceland Psychiatry is seeking a compassionate, organized, and dependable Care Coordinator / Case Manager to join our behavioral health treatment team. The Care Coordinator will play an important role in helping clients successfully navigate treatment by providing ongoing communication, coordination, resources, and practical support throughout their time in the program.
This position is ideal for someone who enjoys working directly with people, collaborating with a multidisciplinary treatment team, solving problems, and helping clients overcome barriers to treatment.
The Care Coordinator will work closely with intake staff, therapists, psychiatrists, clients, families/support systems, and community resources to ensure that client needs are communicated and addressed appropriately.
Key Responsibilities
. Client Communication & Support
· Maintain regular communication with clients regarding treatment-related needs, appointments, transportation, and other concerns.
· Serve as a consistent point of contact for clients who need assistance navigating program-related needs.
· Help identify barriers that may interfere with a client’s ability to participate successfully in treatment.
· Assist clients with completing forms, applications, and other necessary documentation.
· Provide appropriate community resources and referrals based on individual client needs.
· Assist clients with connecting to resources related to housing, transportation, employment, financial assistance, medical care, recovery support, and other areas as appropriate.
· Communicate with a client’s support system when clinically appropriate and when proper authorization/consent is in place.
Intake & Admission Support
· Assist with intake and admission duties as needed, including interviewing prospective clients to gather relevant demographic, clinical, and treatment information.
· Assist prospective clients with completing required intake paperwork and documentation.
· Create and maintain accurate patient profiles within the organization's Electronic Health Record (EHR) system.
· Ensure intake information is entered accurately and communicated to the appropriate members of the treatment team.
· Assist with coordinating the transition from the intake process into the appropriate level of care and treatment services.
Transportation Coordination
· Coordinate transportation for clients traveling to and from the treatment facility.
· Schedule and confirm rides as needed.
· Communicate transportation changes, delays, or concerns to clients and appropriate staff.
· Monitor transportation needs and help address barriers that could affect attendance.
Treatment Team Collaboration
· Communicate and collaborate closely with intake staff, therapists, psychiatrists, and other members of the treatment team.
· Provide relevant updates regarding client needs, barriers, transportation, resources, and other case-management concerns.
· Participate in treatment team meetings and other meetings as assigned.
· Help ensure continuity of communication between the client and members of the treatment team.
· Follow appropriate confidentiality, documentation, and release-of-information requirements.
Administrative Responsibilities
· Complete daily administrative and organizational tasks necessary to support the program.
· Maintain accurate and timely documentation of case-management activities and client contacts.
· Assist with scheduling, coordination, tracking, and other program-related duties.
· Maintain organized records and follow established program procedures.
· Assist with miscellaneous administrative projects and tasks as assigned.
· Demonstrate flexibility and willingness to assist with the changing needs of the program.
Qualifications
· Previous experience in behavioral health, case management, care coordination, social services, healthcare, or a related field preferred.
· Strong communication and interpersonal skills.
· Excellent organization and time-management skills.
· Ability to manage multiple tasks and priorities in a fast-paced environment.
· Ability to communicate professionally with clients, families, treatment providers, and community organizations.
· Strong problem-solving skills and the ability to identify practical solutions to client needs.
· Comfortable using computers, email, scheduling systems, electronic health records, and other administrative technology.
· Ability to maintain appropriate professional boundaries and protect confidential client information.
· Reliable attendance and punctuality.
· Ability to work independently while also functioning effectively as part of a multidisciplinary team.
· Compassionate, nonjudgmental, and client-centered approach.
Preferred Qualifications
· Bachelor’s degree in social work, psychology, human services, counseling, healthcare, or a related field.
· Experience working with individuals experiencing mental health or substance use concerns.
· Experience with case management, discharge planning, care coordination, or behavioral health services.
· Familiarity with community resources and social-service systems.
· Knowledge of HIPAA, confidentiality requirements, and behavioral health documentation.

The Care Coordinator must be able to recognize when a client needs additional assistance, communicate those needs to the treatment team, and follow through to help connect the client with appropriate resources.
This is a hands-on position where no two days are exactly the same. The successful candidate will be comfortable balancing client needs, transportation coordination, treatment-team communication, documentation, and administrative responsibilities throughout the day.
