Position Details:


Employment Type: Full-Time, 40 hours per week
Schedule: Monday–Friday 8 AM to 5 PM
Compensation: $23 per hour

Job Knowledge and Comprehension:


Provides specialized treatment, assistance, education, rehabilitation, and support services as part of a comprehensive team to C.A.R.E. program clients.
Completes comprehensive assessments and evaluations necessary for establishing client eligibility for C.A.R.E. services.
Participates as part of a comprehensive team in the treatment planning process and monitors client progress in meeting treatment plan goals and objectives.
Assesses, monitors, locates, and coordinates necessary medical, social, psychiatric, financial, residential, vocational, and daily living needs and services for C.A.R.E. program clients.
Provides emergency services including crisis assessment and intervention to C.A.R.E. program clients.
Works cooperatively with other agencies, organizations, individuals, and community services to develop resources and ensure comprehensive treatment for C.A.R.E. program clients.
Assists in developing, implementing, coordinating, monitoring, reviewing, and updating programming opportunities for the C.A.R.E. program.
Communicates information regarding programming needs, topics, schedules, changes, and updates to C.A.R.E. program staff.
Utilizes community resources in developing and maintaining programming opportunities for the C.A.R.E. program.
Monitors and assesses program and client needs and utilizes this information in developing programming opportunities.
Works closely with Department of Rehabilitation Services staff to ensure efficient and effective provision of services. 
Monitors, reviews and assists in the development and maintenance of the Vocational program budget including payments from the Department of Rehabilitation Services.
Submits and ensures that timely monthly and quarterly reports are submitted to the Department of Rehabilitation Services. 
Performs other duties as assigned consistent with the responsibilities of the position.



Physical Requirements:
Required to sit for extended periods of time. Infrequent light physical effort required, including lifting up to 10 pounds occasionally and up to 25 pounds rarely. The employee frequently is required to sit and reach with hands and arms. Incumbent may be required to work extended periods of time at a computer monitor. Must be able to occasionally operate a motor vehicle.

 
Qualifications
Associate's degree in social sciences, human services, or related field required.
Bachelor’s degree in social work, psychology, or related field preferred.
Valid South Dakota Driver’s License required.
