Bilingual Health Services Care Manager (Russian)

The Health Services Care Manager Mentor works within the framework of the Executive Office of Aging and Independence (AGE) regulations, to assist new staff, provide ongoing training and support staff as they learn to achieve a high quality of care management. Provides a consumer centered approach to assessment, service acquisition, reassessment, and monitoring of the services to assist elders to live independently in the community.

Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week and hybrid work schedule (once training is complete.) No nights or weekends.

Compensation: Starting annual salary range is $53,105.78 - $57,210.01. A 6% increase will be used to recognize the required bilingual skill set in Russian that is required for this role.Β

Job Activities

Job activities of the position are below. Additional duties may be assigned as required.

Top Key Activities:

  • Work in conjunction with the Director of Home Care, Training Coordinator, and Home Care Team Managers to assist, train and orient new staff to attain a high level of care
  • Provide case management to assigned caseload while meeting AGE timelines and documentation standards
  • Develop, authorize, implement and monitor a care plan within budget guidelines to meet identified need(s) during periodic reassessments by telephone, video conferencing, and by visiting the consumer in their home
  • Coordinate community-based services, and ensure access to public benefits with consumers, families, caregivers, and service providers.
  • Participate in collaborative team model and regular supervision meetings

Other Activities:

  • Cross collaboration with all departments for best practices
  • Attend AGE and MVES training sessions

Qualifications:

  • A Bachelors' degree from an accredited school in Social Work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, or Gerontology plus at least two years of experience in counseling, casework or case management.
  • Fluent in English and Russian.
  • Completed successful introductory period and met compliance measures for the last six months
  • Excellent interpersonal skills with demonstrated ability to communicate across all levels, including staff, family and external partners
  • Ability to maintain confidentiality while working with sensitive information or in sensitive situations
  • Highly resourceful team player, with the ability to work effectively independently or as a team
  • Demonstrates a willingness to take on new tasks with a general attitude that no task is too small, impossible, or cannot be improved
  • Expertise in the following platforms: Microsoft Office (Word, Outlook, Teams, Excel, and PowerPoint), Adobe Pro, Zoom and other web-based applications

Physical Requirements of the Job:

  • Ability to lift, push, pull and carry objects weighing up to 25 pounds
  • Ability to climb up and down stairs unassisted
  • Ability to travel to the community to perform visits at clients' homes
  • Work requires regular standing, stooping, and bending
