SUMMARY

In collaboration with the Director of Crisis Services and the Mobile Crisis Response Supervisors, this position provides administrative support to the Mobile Crisis Response team to triage crisis response case assignments, support the facilitation of hospital placements and discharge appointments, and help organize, aspects of the Mobile Crisis Response and SASS services provided by Pillars Community Health. This position will also perform limited case management activities and have direct client contact.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Develops and maintains resources for community linkages related to MCR services
  • Monitors Mobile Crisis Response webpage and accepts and facilitates transfers on a daily basis.
  • Coordinates all Crisis program client assignments for post-crisis follow-up
  • Assists with facilitating the completion of consent forms with the client's family during the crisis and/or after the crisis response, as needed. This may include traveling to the client's location.
  • Attends hospital and other community stakeholder meetings as needed
  • Works with hospital discharge planners on a daily basis to triage the follow-up needs of clients discharging from the hospitals.
  • Gathers and inputs data into electronic health records according to department needs and compliance requirements.
  • Maintains various logs and electronic client records in accordance with department and agency policies to help track and monitor key performance indicators and program outcomes
  • Outreaches clients and client families and maintains all logs and tracking systems according to department needs
  • Follows-up with clients and their family to identify and discuss resources, gaps in resources, and provides information about continued treatment options. Client meetings may occur at the office, the client's home, or a community location.
  • Collaborates with other intake staff throughout the organization (Psychiatry, Outpatient Mental Health, etc) in processing client need for services.
  • Assist with continued post-crisis placement efforts in collaboration with other clinicians. In situations involving greater than 48 hours for placement, travels to the hospital to meet with the client, presents the assessment, gathers information about changes, communicates with the ER staff and physician. Discusses any changes, the reassessment, and safety plans with the Director.
  • Participates in organizational meetings, in-services, monthly department meetings, and weekly staff meetings.
  • As needed, maintain a small caseload to support case management needs of clients post-crisis services
  • Other duties as assigned.

 

QUALIFICATIONS

EDUCATION AND EXPERIENCE

Bachelor's degree in counseling, psychology, social work, marriage and family therapy or related field required. Equivalent combination of education and experience may be considered in lieu of degree.

 

1-2 years of direct client case management experience in social service, behavioral health, community health outreach, or related field required. Prior experience coordinating health care for clients/patients is preferred. Customer service experience required.

 

LANGUAGE SKILLS

Bilingual (English/Spanish) required.

Seeking candidate with a positive, caring, and compassionate attitude toward mental health and those living with mental illness. Must be able to problem solve and make quick decisions during intake calls. Must have excellent organizational and communication skills. Candidate must demonstrate initiative and be a self-starter. Candidate must be able to collaborate with other health care providers and work within different systems. Must be able to communicate effectively with emergency room (ER) staff and other mental health providers. Intermediate computer skills required.

 

OTHER

Must maintain a safe driving record and have a current valid Illinois Drivers License. Personal vehicle needed for travel to/from office and community-based interventions. Mileage for authorized work-related purposes is reimbursable.

PAY & BENEFIT INFORMATION

$18.50 - $22.50 per hour*
