Pay: $25.00 – $27.00 per hour (based on degree and years of experience)

Job Summary

We are seeking a motivated and organized Care Coordinator to join our behavioral health team in Houston, Texas. This office-based role is ideal for an experienced professional who is passionate about helping children, families, and individuals access quality behavioral health services.

The Care Coordinator will be responsible for completing assessments, coordinating services, supporting clients through case management, and ensuring documentation compliance.

Required Qualifications

• Must meet QMHP qualifications under Texas standards
• Currently certified in CANS and/or ANSA
• Bilingual (English/Spanish)
• Minimum Bachelor’s Degree in Human Services, Social Work, Psychology, Counseling, Sociology, or related field
• Previous case management experience required
• Previous CANS/ANSA completion experience required
• Strong organizational and communication skills
• Ability to work independently and manage deadlines

Responsibilities

• Complete CANS/ANSA assessments accurately and timely
• Coordinate care and connect clients to community resources
• Maintain communication with families, providers, and referral sources
• Assist with service planning and treatment coordination
• Ensure documentation meets Medicaid, Texas, and agency standards
• Monitor client progress and identify service needs
• Maintain confidentiality and HIPAA compliance
• Support office operations and intake workflow as needed

Preferred Qualifications

• Medicaid behavioral health experience
• Knowledge of Texas community mental health systems
• Experience working with children and families
• Experience using electronic health records systems
